RECOMMENDATION:

Adopt a resolution authorizing three agreements with Ravenswood Family Health Center for the term of January 1, 2018, through December 31, 2020, to provide:
A) Primary care services for homeless individuals for an amount not to exceed $321,300; and
B) Dental services for homeless individuals for an amount not to exceed $164,175; and
C) Enabling and supportive services for homeless individuals for an amount not to exceed $291,000.

BACKGROUND:
The San Mateo County Health System has been awarded the Health Care for the Homeless/ Farmworker Health (HCH/FH) grant from the Bureau of Primary Health Care, Health Services and Resources Administration of the US Department of Health and Human Services. The grant funds San Mateo Medical Center's (SMMC) ongoing HCH/FH program, which is designed to assist homeless and farmworker adults, families, and youth with access to and utilization of SMMC's continuum of care and other medical, health and support services. HCH/FH has contracted with Ravenswood Family Health Center (RFHC) since 2008, to provide various medical and health services.
DISCUSSION:
In September 2017, HCH/FH issued a Request for Proposals (RFP) for providers to deliver health and health related services to the homeless and farmworker populations of San Mateo County. Upon review of the RFP submissions, the HCH/FH Program Co-Applicant Board selected RFHC to provide some of these services due to its proposed quality and volume of services, as well as the benefit to the County's homeless population.
