BACKGROUND:

Transitional Housing Placement Plus (THP-Plus) program originated in 2006 as a collaboration between John Burton Foundation, California Department of Social Services, and Corporation for Supportive Housing. The goal of Transitional Housing Placement Plus (THP-Plus) program is to provide a safe living environment while helping former foster youth, now young-adults, achieve self-sufficiency by learning life skills in preparation for completely exiting the foster care support system.

San Mateo County Human Services Agency (HSA) issued a request for proposals (RFP) in May 2017 seeking a provider for Transitional Housing Placement Plus program (THP-Plus) services for former foster youth ages 18 through 24 under the scattered/host site models. HSA selected Unity Care Group, Inc. (UCG) as the provider that best meets the needs of the County based on their proposed approach and demonstration of successfully providing THP-Plus services in other counties.

 

DISCUSSION:

THP-Plus services are designed to provide former foster youth with housing assistance, financial and educational tools, and other supportive resources needed to help former foster youth achieve self-sufficiency, permanency, and lifelong connections. With your Board’s approval, this agreement will provide THP-Plus services to former foster youth over the next eight months under the scattered/host site models. This is a new provider for THP-Plus services.

UCG will provide services to include supports such as case management, educational guidance, employment counseling, finance management, housing search and placement, and assistance in reaching goals identified in the young-adult’s Transitional Independent Living Plan (TILP). UCG will also provide aftercare services to former foster youth, including those who have participated in the THP-Plus program and those who have not, to include short-term case management and time-limited payments to help young-adults remain stable, housed, in school and/or employed.

The resolution contains the County’s standard provisions allowing amendments of the County’s fiscal obligations by a maximum of $25,000 (in aggregate).

The agreement and resolution have been reviewed and approved by County Counsel as to form.

Approval of this agreement contributes to the Shared Vision 2025 outcome of a Prosperous Community by providing a safe living environment while helping former foster youth, now young-adults, achieve self-sufficiency by learning life skills in preparation for completely exiting the foster care support system. It is expected that a minimum of 23 former foster youth (young-adults) will participate in the THP-Plus program under this Agreement, of which approximately 16 will be single and 7 will be parenting a child(ren). It is anticipated that 90% of former foster youth receiving THP-Plus services will complete their GED or maintain college placement, and that 90% of youth not enrolled full-time in post-secondary education will increase their earned income by 20% as a result of the THP-Plus program.

FISCAL IMPACT:

The term of this agreement is November 1, 2017 through June 30, 2018. The total amount of the agreement is $658,780, and is funded 100% by Child Welfare Services revenue through the State Transitional Housing Placement - Plus (THP-Plus) allocation. There is no Net County Cost. Appropriations have been included in the FY 2018-19 Recommended Budget.
