BACKGROUND:
Transitional Housing Placement Plus (THP-Plus) program originated in 2006 as a collaboration between John Burton Foundation, California Department of Social Services, and Corporation for Supportive Housing. The goal of Transitional Housing Placement Plus (THP-Plus) program is to provide a safe living environment while helping former foster youth, now young-adults, achieve self-sufficiency by learning life skills in preparation for completely exiting the foster care support system.

San Mateo County Human Services Agency (HSA) issued a request for proposals (RFP) in May 2017 seeking a provider for Transitional Housing Placement Plus program (THP-Plus) services for former foster youth ages 18 through 24 under the scattered/host site models. HSA selected Unity Care Group, Inc. (UCG) as the provider that best meets the needs of the County based on their proposed approach and demonstration of successfully providing THP-Plus services in other counties.

DISCUSSION:
THP-Plus services are designed to provide former foster youth with housing assistance, financial and educational tools, and other supportive resources needed to help former foster youth achieve self-sufficiency, permanency, and lifelong connections. With your Board's approval, this agreement will provide THP-Plus services to former foster youth over the next eight months under the scattered/host site models. This is a new provider for THP-Plus services.
