BACKGROUND:

The County operates health care facilities collectively known as the San Mateo Medical Center (SMMC), which provides care to San Mateo County residents who have little or no access to other sources of medical care and serves as the public system hub of the health care safety net.

 

In August 2015, your Board approved an agreement with Puente de la Costa Sur (Puente) to extend such services to those in remote and not always easily accessible locations to certain members of the San Mateo County South Coast communities.

 

DISCUSSION:

Puente wishes to use the approved Measure K funds to continue to provide approximately 9,000 outreaches per year to low income, hard-to-reach residents in the South Coast communities. The specific target population consists of residents eligible for or enrolled in Medi-Cal, Medicare, Healthy Kids, or the Access to Care for Everyone programs. In addition, Puente will also conduct health education in small group settings at geographically accessible locations such as ranches, farms, or community locations; promote preventive health campaigns; help connect clients with services; and otherwise help manage cases for those served by Puente.

 

The agreement and resolution have been reviewed and approved by County Counsel as to form.  This agreement is coming to your Board late due to protracted negotiations.

 

The resolution contains the County’s standard provisions allowing amendment of the County fiscal obligations by a maximum of $25,000 (in aggregate).

The agreement contributes to the Shared Vision 2025 outcome of a Healthy Community by providing San Mateo County residents who frequently do not have adequate access to healthcare and other services. It is anticipated that there will be an average of 750 client contacts per month.

FISCAL IMPACT:

The term of the agreement is July 1, 2017 through June 30, 2019. The amount of the agreement is not to exceed $367,824 for the two-year term. Funds in the amount of $183,912 are included in the SMMC FY 2017-18 Approved Recommended Budget. Similar budget arrangements will be made for future years.

 

This payment provisions in this contract differ from the previous agreement due to a 3% increase in the not to exceed amount.

 

Expenses at SMMC are covered by fees for services or third-party payors whenever possible. The portion of expenses for services provided to the medically indigent or to those covered by programs that do not meet the full costs of care are covered by the County’s General Fund contribution to SMMC, and are within the existing annual appropriation.
