BACKGROUND:

San Mateo County is responsible for providing necessary medical care to its medically indigent population.  This care includes disorders of the kidney, urethral tract, and bladder, and the management and supervision of those services.

DISCUSSION:

Dr. David Martin has served as San Mateo Medical Center’s (SMMC) Urology Medical Director since October 2012. He has agreed to continue as Medical Director, in addition to providing four half-day urology clinics per week and two half-day surgery blocks per week. 

 

County Administrative Memorandum B-1 provides that contracts for physicians are exempt from the Request for Proposals process.

 

The resolution contains the County’s standard provisions allowing amendment of the County’s fiscal obligations by a maximum of $25,000 (in aggregate).

 

The agreement and resolution have been reviewed and approved by County Counsel as to form. 

 

The agreement contributes to the Shared Vision 2025 outcome of a Healthy Community by providing access to urology services to residents of San Mateo County.  It is anticipated that 100% of all patients diagnosed with kidney stones will receive lithotripsy treatment within 24 hour of the diagnosis.

FISCAL IMPACT:

The term of the agreement is October 1, 2017 through September 30, 2019.  The amount of the agreement is not to exceed $1,050,000 for the two-year term.  Funds in the amount of $394,000 are included in the SMMC FY 2017-18 Approved Recommended Budget.  Funds in the amount of $525,000 will be included in the SMMC FY 2018-19 Recommended Budget.  Similar arrangements will be made for future years. 

 

The payment provisions of the contract are different from the prior contract because of a 7% increase over two years (3.5% per year) in the Medical Group Management Association urology rates upon which this contract is based

 

Expenses at SMMC are covered by fees for services or third-party payors whenever possible.  The portion of expenses for services provided to the medically indigent or to those covered by programs that do not meet the full costs of care are covered by the County’s General Fund contribution to SMMC, and are within the existing annual appropriation.
