BACKGROUND:

San Mateo County is responsible for providing necessary medical care to its medically indigent population, as well as access to services to its general population.  Dr. Buckley has been providing ophthalmology services at San Mateo Medical Center (SMMC) for more than twenty years and will continue to provide, at a minimum, the same level of service as he has under previous contracts with the County.

 

DISCUSSION:

The County’s need for ophthalmology services is increasing due to the aging of its patient population. SMMC will continue to meet this growing need through contracts with Dr. Buckley and other ophthalmologists. Dr. Buckley has agreed to provide the necessary surgery, clinic and call coverage for County patients.

 

County Administrative Memorandum B-1 provides that contracts for physicians are exempt from the Request for Proposals process.

 

The agreement and Resolution have been reviewed and approved by County Counsel as to form.

 

The Resolution contains the County’s standard provisions allowing amendment of the County fiscal obligations by a maximum of $25,000 (in aggregate).

 

The agreement contributes to the Shared Vision 2025 outcome of a Healthy Community by providing San Mateo County residents access to ophthalmology services. It is anticipated that 100% of all cataract surgery services provided will adhere to the infection control standards as defined by the Joint Commission and Title XXII of the State of California, Code of Regulations, which will result in reduced risk of healthcare-associated infections.

FISCAL IMPACT:

The term of the agreement is September 1, 2017 through August 31, 2019. The amount of the agreement is not to exceed $500,000 for the two year term. Funds in the amount of $210,000 are included in the SMMC FY 2017-18 Approved Recommended Budget. Funds in the amount of $250,000 will be included in the SMMC FY 2018-19 Recommended Budget. Similar arrangements will be made for future years.

 

The payment provisions have increased because the 2016 Report of Physician Compensation and Production Survey provided by the Medical Group Management Association, upon which the pay for this contractor is based, increased by 6% compared to the 2014 Report.

 

Expenses at SMMC are covered by fees for services or third-party payors whenever possible.  The portion of expenses for services provided to the medically indigent or to those covered by programs that do not meet the full costs of care are covered by the County’s General Fund contribution to SMMC, and are within the existing annual appropriation.
