BACKGROUND:

Since September 2014, Dr. Renata Jarosz has provided a musculoskeletal clinic and physical medicine (physiatry) services for San Mateo Medical Center (SMMC).  The main purpose of the physiatry clinic is to diagnose and treat patients with complex musculoskeletal pain syndromes and to triage referrals to orthopedic surgery and neurosurgery services.  The screen/triage process ensures that only surgical patients are referred to orthopedics and neurosurgery, and physical medicine patients are treated non-surgically.  Because patients are now appropriately sorted, patients in both the surgical and non-surgical settings are seen in a shorter amount of time.

 

Dr. Jarosz has provided excellent coordination of physiatry, neurosurgery, orthopedic and physical therapy services.

 

DISCUSSION:

Dr. Jarosz will continue to provide physiatry services five days a week to SMMC patients.  In addition, Dr. Jarosz also serves as Medical Director for the Physical and Occupational Therapy Departments.

 

The Resolution contains the County’s standard provisions allowing amendment of the County fiscal obligations by a maximum of $25,000 (in aggregate).

 

The agreement and Resolution have been reviewed and approved by County Counsel as to form.

 

County Administrative Memorandum B-1 provides that contracts for physicians are exempt from the Request for Proposals process.

 

The agreement contributes to the Shared Vision 2025 outcome of a Healthy Community by providing San Mateo County residents with access to basic physiatry services.  It is anticipated that the third to next available orthopedic appointment will be less than or equal to 10 days.

FISCAL IMPACT:

The term of the agreement is September 1, 2017 through August 31, 2018. The amount of the agreement is not to exceed $326,000 for the one-year term. Funds in the amount of $272,000 are included in the SMMC FY 2017-18 Recommended Budget.  Funds in the amount of $54,000 will be included in the SMMC FY 2018-19 Recommended Budget. 

 

The payment provisions for this contract are different from the previous contract due to the rate change increase for physiatrists, as per the benchmarking organization Medical Group Management Association (MGMA). This contract represents a 3% increase over the previous contract, to reflect current MGMA rates. Also included in this contract amount is $3,000 per month for Dr. Jarosz’s Medical Director duties.

 

Expenses at SMMC are covered by fees for services or third-party payors whenever possible. The portion of expenses for services provided to the medical indigent or to those covered by programs that do not fully meet the costs of care are covered by the County General Fund contribution to SMMC, and are within the existing annual appropriation.
