BACKGROUND:
San Mateo County Health has been awarded the Health Care for the Homeless/Farmworker Health (HCH/FH) grant from the Bureau of Primary Health Care, Health Services and Resources Administration of the US Department of Health and Human Services. The grant funds San Mateo Medical Center's (SMMC) ongoing HCH/FH program, which is designed to assist homeless and farmworker adults, families, and youth with access to and utilization of SMMC's continuum of care and other medical, health and support services. In July 2024, the HCH/FH program completed a Request for Proposals for case management and care coordination services.

DISCUSSION:
HCH/FH requested proposals for a range of case management and enabling services for homeless individuals, farmworkers, and their dependents. These services include 1. street medicine case management and care coordination, 2. intimate partner violence, domestic violence, human trafficking case management, 3. medical care coordination for newly housed individuals, 4. Promotores de Salud, 5. MyChart orientation, 6. health insurance assistance, 7. transportation, and 8. nutrition education.
