Adopt a resolution authorizing an agreement with Coastside Hope to provide care coordination services for the term of January 1, 2025 through December 31, 2027, in an amount not to exceed $424,233.

BACKGROUND:
San Mateo County Health has been awarded the Health Care for the Homeless/Farmworker Health (HCH/FH) grant from the Bureau of Primary Health Care, Health Services and Resources Administration of the US Department of Health and Human Services ("HHS"). The grant funds San Mateo Medical Center's (SMMC) ongoing HCH/FH program, which is designed to assist homeless and farmworker adults, families, and youth with access to and utilization of SMMC's continuum of care and other medical, health and support services. In July 2024, the HCH/FH program completed a Request for Proposals for case management and care coordination services.
DISCUSSION:
HCH/FH requested proposals for a range of case management and enabling services for homeless individuals, farmworkers, and their dependents. Coastside Hope was selected to provide care coordination services, health insurance assistance, and transportation assistance due to their extensive experience and demonstrated success in providing essential safety net services to residents of the Midcoast region for over 45 years. Coastside Hope offers a comprehensive range of services addressing critical areas such as economic, food, and housing insecurity. These services include multiple food programs, housing and homeless prevention services, emergency financial assistance, transportation support, tax preparation, and case management.
