In July 2024, the HCH/FH program completed a Request for Proposals (RFP) for case management and care coordination services.

LifeMoves was elevated for their 13+ years of experience providing case management services to individuals experiencing homelessness in San Mateo County. Their deep expertise and long-standing relationships with clients have fostered trust and led to consistently positive outcomes, such as stable housing and improved well-being. Their proven track record and commitment to tailored, impactful services make them a reliable partner in addressing homelessness in the community.
