Adopt a resolution authorizing an agreement with Health Plan of San Mateo to fund two positions for a three-year period to support the implementation and expansion of Enhanced Care Management services provided by Bridges to Wellness for the term of September 1, 2024 through December 31, 2027 in an amount not to exceed $886,056.

BACKGROUND:
The California Department of Health Care Services is transforming Medi-Cal through CalAIM initiatives to build a more coordinated, person-centered, and equitable health system. As part of that transformation, the Health Plan of San Mateo (HPSM) contracts with County Health's Public Health, Policy & Planning to provide Enhanced Care Management services to medically at-risk persons experiencing homelessness, mental health challenges, and substance use disorder through the Bridges to Wellness program.
DISCUSSION:
HPSM seeks to grant Bridges to Wellness, through the CalAIM Incentive Payment Program, monies to create sustainable infrastructure and to expand services to the justice-involved population. Funds will be used for two limited term positions to support the Enhanced Care Management populations. Positions are intended to build technical capacity, develop best practices in alignment with state guidance and assess the need of the justice-involved population.
