BACKGROUND:
The Primary Care Engagement Pilot is an innovation project by San Mateo Medical Center (SMMC). The project aims to expand access to those currently unengaged with primary care through community-based co-design. The project originates from SMMC's strategic intent to move towards a value-based care future and the recognized need to improve primary care access. The project was developed in alignment with both SMMC and San Mateo Health Commission dba Health Plan of San Mateo (HPSM) leadership.

On February 5, 2025, HPSM approved funding for a Primary Care Engagement Pilot through SMMC Innovation Lab.

DISCUSSION:
In accepting the $300,000 funding, SMMC will complete the ethnographic research phase, co-develop a Care Team Model based on research results, and report out findings to HPSM. This agreement is going to this Board late due to prolonged negotiations with the vendor.

The County Attorney has reviewed and approved the resolution and agreement as to form.

It is anticipated that SMMC will report out to HPSM by the due dates listed in the agreement 100% of the time.
