Adopt a resolution authorizing an amendment to the agreement with Puente de la Costa Sur to provide enabling services, extending the term of the agreement through June 30, 2021 and increasing the amount payable by $123,250 to an amount not exceed $673,750.

BACKGROUND:
In September 2017, the Health Care for the Homeless / Farmworker Health Program (HCH/FH) completed a Request for Proposals (RFP) for enabling services. Enabling services are non-clinical services that do not include direct patient services, which enable individuals to access health care and improve health outcomes. Examples of enabling services include: case management, referrals, translation/interpretation, transportation, eligibility assistance, health education, environmental health risk reduction, health literacy, and outreach.
Puente de la Costa Sur was chosen for their efficiency in delivering enabling services to the population they serve. The agreement has been amended previously, most recently by your Board on April 9, 2019, increasing the amount by $367,000 to an amount not to exceed $550,500. This agreement is funded through a grant received from the U.S. Department of Health and Human Services, Health Resources, and Services Administration (HRSA).
DISCUSSION:
HCH/FH had planned to release an RFP for enabling services in the second quarter of 2020, in order to have new agreements in place for enabling services by January 1, 2021.
