BACKGROUND:
In September 2017, the Health Care for the Homeless / Farmworker Health Program (HCH/FH) completed a Request for Proposals (RFP) for enabling services. LifeMoves was chosen to provide enabling services, which are non-clinical services that enable individuals to access health care and improve health outcomes. Enabling services include case management, referrals, translation/interpretation, transportation, eligibility assistance, health education, environmental health risk reduction, health literacy, and outreach.
The agreement has been amended previously; most recently, your Board approved an amendment extending the term of the agreement through December 31, 2020, in an amount not to exceed $890,280. This agreement is funded through a grant received from the U.S. Department of Health and Human Services, Health Resources and Services Administration (HRSA).
DISCUSSION:
HCH/FH had planned to release an RFP for enabling services in the second quarter of 2020, in order to have new agreements in place by January 1, 2021. However, due to the COVID-19 pandemic and the need to continue providing services to individuals experiencing homelessness, this was not feasible. Therefore, the San Mateo Medical Center (SMMC) proposes that your Board approve this amendment, extending the term of the agreement through June 30, 2021 and increasing the amount by $154,900 to an amount not to exceed $1,045,180.
The agreement and resolution have been reviewed and approved by County Counsel as to form.
The resolution contains the County’s standard provisions allowing further amendment of the County fiscal obligations by a maximum of $25,000 (in aggregate).
The County’s Contract Compliance Committee has approved a waiver request for a forty-two months term for this agreement.
It is anticipated that a minimum of 180 unduplicated individuals experiencing homelessness will receive enabling services.

FISCAL IMPACT:
The term of the amended agreement with LifeMoves is January 1, 2018 through June 30, 2021. The amount of the agreement is not to exceed $1,045,180 for the three-and-a-half-year term. The amendment increases the amount payable under the agreement by $154,900. Funds in the amount of $154,900 are included in the SMMC FY 2020-21 Adopted Budget.
Expenses for this agreement are completely funded by the ongoing Section 330 Public Health Act Community Health Center grant award from HRSA.
