BACKGROUND:

Beginning in 2003, the Administration on Aging, now the Administration for Community Living, merged with the Centers for Medicare and Medicaid Services (CMS), to promote and fund Aging and Disability Resource Centers, which were designed to help individuals learn more about their community long-term services and supports (LTSS) options to promote the receipt of community-based LTSS in the most integrated setting possible.

 

Instead of LTSS, California has opted to use the modified title of Aging and Disability Resource Connection (ADRC) to reflect the principle of No Wrong Door (NWD), as opposed to a “single entry point” concept. This means that multiple agencies at the State and local level coordinate to create a simplified process through which individuals of all ages, abilities, and incomes can obtain unbiased information and one-on-one counseling on the options available in their communities. This allows individuals to make informed decisions about services that best meet their needs, regardless of where they enter the system. Consumers also receive assistance in accessing services, including help with applications for various public and private programs.

 

In 2019, California Senate Bill 80 established the first State ADRC Infrastructure Grants program. California Department of Aging (CDA) awards grants to qualified Area Agencies on Aging (AAA) and Independent Living Centers to complete the planning and application process for designation and approval to operate as an ADRC program, as well as to aid designated ADRC programs in expanding and improving services.

 

DISCUSSION:

County Health’s Aging and Adult Services (AAS) is the County’s local AAA and will be responsible for granting and the ADRC funds to the Center for Independence of Individuals with Disabilities (CID), and reporting on CID’s spending of the ADRC funds.  CID is a private, nonprofit corporation located in the City of San Mateo that provides services to more than 2,000 individuals with disabilities every year. CID is one of 29 Independent Living Centers in the State of California, but is the only ILC in the County. A waiver of the request for proposal process is sought because CID is the sole source for ADRC services and is the only qualifying grant partner in San Mateo County.

Upon this Board’s approval of this agreement, AAS will enter into an agreement with CID in order to grant the ADRC funds to CID. 

 

Using the ADRC funding, CID will provide the following services:

 

1.                     Information and referral services at hours that are convenient for the public

 

2.                     Counseling to consumers concerning available LTSS programs and 

public and private benefits programs

 

3.                     Short-term care services coordination

4.                     Assistance with transitions from hospitals to home and from skilled nursing facilities to the community

 

This first grant from CDA, in the amount of $245,448, is for project planning and infrastructure development by ADRC. CID will use these funds to hire staff members and build a database to track and report referrals and other services provided to consumers. The ADRC will receive annual funding through CDA to support its operations.

 

This agreement is coming to this Board late because AAS received CDA confirmation of the grant recently.

 

The agreements and resolution have been reviewed and approved by the County Attorney as to form.

 

The resolution contains the County’s standard provisions allowing amendment of the County fiscal obligations.

 

It is anticipated that AAS will complete the application process for State ADRC designation and execute an ADRC work plan for implementation of a local NWD and ADRC program by Fiscal Year 2023-24.

FISCAL IMPACT:

The term of the provider agreement July 1, 2022 through June 30, 2023. The amount of the agreement is not to exceed $245,448 for the one-year term. Funds for these services were included in the AAS FY 2022-23 Adopted Budget. Federal funds will cover 100% of the costs. There is no Net County Cost impact by adopting these agreements.
