BACKGROUND:

San Mateo Medical Center (SMMC), Aging and Adult Services (AAS), and Behavioral Health and Recovery Services (BHRS) have a continued need for emergent and non-emergent patient transport services. Non-emergent transportation services are needed for patients with mobility issues to transfer between facilities, routine medical appointments, and post-discharge transportation. In June 2024, SMMC completed a Request for Proposals for non-emergent patient transport services and Royal Ambulance, Inc. (Royal) was elevated.

 

DISCUSSION:

SMMC is recommending entering into a new agreement with Royal due to their cost, quick turnaround time, and vast variety of special services offered. Royal has agreed to provide patient transport services for the term of November 1, 2024, through October 31, 2027, in an amount not to exceed $ 1,907,689.

 

County Attorney has reviewed and approved the resolution and agreement as to form.

 

The resolution contains the County’s standard provisions allowing amendment of the County fiscal obligations by a maximum of $25,000 (in aggregate).


It is anticipated that 100% of pre-scheduled transports shall arrive within 15 minutes of scheduled pickup time.

EQUITY IMPACT:

Royal’s patient transportation services will positively impact equitable health outcomes because transportation services at SMMC, a public safety net hospital, addresses community need. SMMC’s patients span a wide spectrum of races, ethnicities, gender, and gender-identities. Within the past 18 months, the largest cohorts of specialty care patients at SMMC were female (56%), Hispanic or Latino (58%), and primarily Spanish speaking (55%).

 

FISCAL IMPACT:

The term of the agreement is November 1, 2024, through October 31, 2027. The amount of the agreement is not to exceed $1,907,689 for the 3-year term. Funds in the amount of $368,375 are included in the SMMC FY 2024-25 Recommended Budget. Funds in the amount of $33,333 will be included in the AAS FY 2025-26 Recommended Budget. Funds in the amount of $22,222 will be included in the BHRS FY 2025-26 Recommended Budget.

 

The payment provisions and levels of services in this contract differ from that of the previous contract, due to an increase in the number of Basic Level Services, Critical Care Transport Services, long distance transports, bariatric transport needs, non-emergent transports and the annual rate adjustment set forth by the Centers for Medicare and Medicaid Services Annual Inflation Factor. The total value of the new contract represents a 150% increase compared to that of the previous contract.

 

Expenses at SMMC are covered by fees for services or third-party payors whenever possible. The portion of expenses for services provided to the medically indigent or to those covered by programs that do not meet the full costs of care is covered by the County’s General Fund contribution to SMMC and is within the existing annual appropriation.
