Adopt a resolution authorizing amendment to the agreement with StarVista to provide enabling services, extending the term of the agreement through June 30, 2021 and increasing the amount payable by $96,250 to an amount not to exceed $426,250.

BACKGROUND:
In September 2017, the Health Care for the Homeless / Farmworker Health Program (HCH/FH) completed a Request for Proposals (RFP) for enabling services as part of a grant received from the Bureau of Primary Health Care of the US Health Services and Resource Administration (HRSA). Enabling services are non-clinical services that do not include direct patient services, which enable individuals to access health care and improve health outcomes. Examples of enabling services include: case management, referrals, translation/interpretation, transportation, eligibility assistance, health education, environmental health risk reduction, health literacy, and outreach.
As a result of the RFP, StarVista was selected to conduct enabling services - specifically outreach, engagement, and adult therapeutic services - to farmworkers and individuals experiencing homelessness in San Mateo County.
DISCUSSION:
HCH/FH proposes that your Board approve an amendment extending the term of the current agreement by six months, through June 30, 2021, with an associated increase in funding, to ensure continuity of services during the COVID-19 pandemic. This will allow San Mateo Medical Center (SMMC) time to issue RFP for enabling services in the first quarter of 2021. Owing to the COVID-19 pandemic, the County was not able to issue an RFP in the time frame originally contemplated.
The amendment to the agreement and resolution have been reviewed and approved by County Counsel as to form.
The resolution contains the County’s standard provisions allowing further amendment of the County fiscal obligations by a maximum of $25,000 (in aggregate).
It is anticipated that at least 145 farmworkers and adults experiencing homelessness will receive enabling services.

FISCAL IMPACT:
The term of the amended agreement with StarVista is January 1, 2019 through June 30, 2021. The amount of the agreement is not to exceed $426,250 for the two-and-a-half-year term. The amendment increases the amount payable under the agreement by $96,250. Funds in the amount of $96,250 are included in the SMMC FY 2020-21 Adopted Budget.
Expenses for this agreement are completely funded by the ongoing Section 330 Public Health Act Community Health Center grant award from HRSA.
