BACKGROUND:

In 2018, Behavioral Health and Recovery Services (BHRS) identified a need to fill a service gap for Severely Mentally Ill (SMI) adult clients that are diagnosed with moderate to severe eating disorders, as defined in the DSM-5, that require a more specialized and intensive level of treatment. At the time, no BHRS clinic or program effectively addressed the comprehensive behavioral, dietary, and medical needs associated with moderate/severe eating disorders. BHRS reached out to several local providers and Cielo House Inc. (Cielo House) was one of two providers that met the requirements to provide comprehensive and specialized treatment for clients diagnosed with eating disorders such as: anorexia, bulimia, binge eating disorder, and eating disorders not otherwise specified. 

 

In July 2018, BHRS conducted an open and fair process to select a provider through obtaining three quotes. Cielo House submitted a quote and BHRS entered into an agreement with Cielo House for the term of July 26, 2018 through June 30, 2020, in an amount not to exceed $300,000. Residential treatment services are exempt from the Request for Proposal process under the B-1 Administrative Memorandum.

 

DISCUSSION:

Cielo House provides Partial Hospitalization and Residential Treatment services for SMI adult clients that have moderate to severe eating disorders and are in need of specialized treatment. Ongoing services include nutritional assessments, comprehensive treatment plans, close supervision, dietary counseling, psychiatry, and individual/family counseling. There is a continued need for these specialized services to support clients with eating disorders in their recovery.

 

In addition to the services described above, Cielo House will also start providing intensive outpatient program services and outpatient treatment services with this new agreement. 

 

During the COVID-19 health crisis, Cielo House has been maintaining service expectations through utilizing online telehealth platforms.  Telehealth services are now a state recognized and billable option to deliver mental health services.

 

The Resolution contains the County’s standard provision allowing amendment of the County’s fiscal obligations by a maximum of $25,000 (in aggregate).

 

The agreement and Resolution have been reviewed and approved by County Counsel as to form. This agreement is late due to prolonged contract negotiations. The agreement is on the Continuing Resolution.

 

It is anticipated that 50% of clients receiving services will not require hospitalization and no more than one Psychiatric Emergency Services (PES) visit.

FISCAL IMPACT:

The term of the agreement is July 1, 2020 through June 30, 2022. The amount of the agreement is not to exceed $400,000 for the length of the term. Funds in the amount of $150,000 are included in the BHRS FY 2020-21 Recommended Budget. Similar arrangements will be made for future years. Of the FY 2020-21 amount, it is anticipated that $200,000 will be funded by 1991 Realignment funds. There is No Net Cost.

 

The not to exceed amount of this agreement is 23% more than the previous agreement, due to inclusion of the new intensive inpatient and outpatient services.
