Adopt a resolution authorizing an agreement with Doximity Inc. to provide an enterprise on-call scheduling application and a compliant telehealth platform for the term of May 6, 2025 through May 5, 2030, in an amount not to exceed $675,000.

BACKGROUND:
In December 2022, this Board approved an agreement with Epic to replace the current Electronic Health Record (EHR). With a new EHR system, San Mateo County Health (SMCH) needs an enterprise on-call and telehealth solution that ensures Health Insurance Portability and Accountability Act (HIPAA) compliance and seamless integration.
DISCUSSION:
To enhance the user experience for our new EHR, it is necessary to incorporate an enterprise on-call and telehealth solution that ensures HIPAA compliance, and that is scalable for a high volume of users across multiple divisions, that has deep integration into the EHR system, and that offers customization of workflows and policies, and a high degree of automation and advanced security feature ensuring efficiency as well as full regulatory compliance.
SMCH/Information Service Department/Health Information Technology completed a Request for Proposals in March 2024 for an On-Call Scheduling Tool and telehealth solution. Doximity, Inc was elevated due to demonstrated integration with Epic, it being staff’s preferred solution, and substantial positive references.
The County Attorney has reviewed and approved the resolution and agreement as to form.
The resolution contains the County’s standard provisions allowing amendment of the County’s fiscal obligations by a maximum of $25,000 (in aggregate).
The Information Services Department has reviewed and approved the IT components of this agreement.
This agreement will allow staff to be more productive in addressing patient issues with the correct provider on call for a given team. It is anticipated that staff will be able to identify and make contact with the appropriate on-call provider, who will quickly and effectively address a patient care issue, over 95% of the time. Additionally, the telehealth platform will be available at least 95% of the time.

COMMUNITY IMPACT:
The purpose of integrating an enterprise on-call scheduler and telehealth solution is to enhance operational efficiency, improve patient care, and provide equal access to healthcare services for all individuals, regardless of their location, socio-economic status, or background. By leveraging technology that aligns with these goals around fairness and effectiveness, San Mateo County Health seeks to create a healthcare environment where timely, high-quality care is accessible to every patient, especially those who are traditionally underserved or marginalized. An enterprise on-call scheduler and telehealth solution will positively impact improved health outcomes by enhancing the safety of services provided to the community, especially at San Mateo Medical Center (SMMC). Demographically, within the past 18 months, the largest cohorts of specialty care patients at SMMC were female (52%), Hispanic or Latino (63%), and primarily Spanish-speaking (55%). SMMC strives to eliminate bias in the medical profession by providing culturally competent training to its physicians and by following up with patients to ensure that they continue to have access to specialized care.
FISCAL IMPACT:
The term of this agreement is May 6, 2025, through May 5, 2030. The contract amount payable under the agreement is not to exceed $675,000 for the five-year term.
Funds in the amount of $150,000 are included in County Health’s FY 2024-25 Adopted Budget. Future costs of $525,000 will be funded by participating SMCH divisions. All patient/client-serving divisions of County Health will contribute to the ongoing costs based on projected use.
