BACKGROUND:

The Innovation Center at San Mateo Medical Center (SMMC) is a unit focused on value-based care and participatory innovations with our patient community. The Innovation Center is conducting a pilot project to improve primary care access to the population that is assigned to SMMC but has thus far not engaged in primary care. The primary care access pilot has been funded for its initial phases by the Health Plan of San Mateo. SMMC discovered this effort will require the expertise of a clinical executive who has done similar work before and is well-versed in innovation methodologies. In March 2025, SMMC completed a Request for Proposals for a value-based care innovation clinical partner.

 

DISCUSSION:

Eva Balint, LLC was elevated due to their ability to get hands-on in the implementation, as evidenced by prior work in developing and running value-based care and innovation programs and projects, and their ability to align with senior leadership and motivate them toward action. Eva Balint, LLC also has knowledge of health plan operations and prior experience working on the payer side and experience with other public agencies. This agreement is going to this Board late due to prolonged negotiations with the vendor.

 

County Attorney has reviewed and approved the resolution and agreement as to form.

 

The resolution contains the County’s standard provisions allowing amendment of the County fiscal obligations by a maximum of $25,000 (in aggregate).


It is anticipated that at least 500 community members and stakeholder participants will be engaged in the project in FY 2025-26, 100% of the innovation care model that will be co-designed with the community in FY 2025-26 will adhere to medical principles, and 100% of innovation activities will correspond to developing the value-based care capabilities within SMMC.

COMMUNITY IMPACT:

Eva Balint LLC’s value-based care innovation clinical partner services at SMMC, a public safety net hospital, addresses community need. A value-based care model addresses health disparities by allocating resources based on patient needs and outcomes, making healthcare more equitable and effective. SMMC’s patients span a wide spectrum of races, ethnicities, gender, and gender-identities. Within the past 18 months, the largest cohorts of specialty care patients at SMMC were female (52%), Hispanic or Latino (63%), and primarily Spanish-speaking (55%).

 

FISCAL IMPACT:

The term of the agreement is May 1, 2025 through April 30, 2026. The amount of the agreement is not to exceed $255,000 for the one-year term. Funds in the amount of $42,500 are included in the SMMC FY 2024-25 Adopted Budget. Funds in the amount of $212,500 will be included in the SMMC FY 2025-26 Recommended Budget.

 

Expenses at SMMC are covered by fees for services or third-party payors whenever possible. The portion of expenses for services provided to the medically indigent or to those covered by programs that do not meet the full costs of care is covered by the County’s General Fund contribution to SMMC and is within the existing annual appropriation.
