Adopt a resolution authorizing an agreement with Coastside Hope to provide care coordination services for the term of January 1, 2025 through December 31, 2027, in an amount not to exceed $424,233.

BACKGROUND:
San Mateo County Health has been awarded the Health Care for the Homeless/Farmworker Health (HCH/FH) grant from the Bureau of Primary Health Care, Health Services and Resources Administration of the US Department of Health and Human Services (“HHS”). The grant funds San Mateo Medical Center’s (SMMC) ongoing HCH/FH program, which is designed to assist homeless and farmworker adults, families, and youth with access to and utilization of SMMC’s continuum of care and other medical, health and support services. In July 2024, the HCH/FH program completed a Request for Proposals for case management and care coordination services.
DISCUSSION:
HCH/FH requested proposals for a range of case management and enabling services for homeless individuals, farmworkers, and their dependents. Coastside Hope was selected to provide care coordination services, health insurance assistance, and transportation assistance due to their extensive experience and demonstrated success in providing essential safety net services to residents of the Midcoast region for over 45 years. Coastside Hope offers a comprehensive range of services addressing critical areas such as economic, food, and housing insecurity. These services include multiple food programs, housing and homeless prevention services, emergency financial assistance, transportation support, tax preparation, and case management.
Additionally, Coastside Hope brings extensive experience in collaborating with local government entities to support vulnerable populations. Their history of service, client-focused approach, and proven collaborative experience make Coastside Hope a trusted partner in delivering critical resources to meet the needs of our program’s target population.
County Attorney has reviewed and approved the resolution and agreement as to form.
The resolution contains the County’s standard provisions allowing amendment of the County fiscal obligations by a maximum of $25,000 (in aggregate).
It is anticipated that a minimum of 800 unduplicated farmworkers or their dependents will receive care coordination or assistance per year.

EQUITY IMPACT:
The HCH/FH program is dedicated to providing comprehensive, culturally competent, and accessible healthcare services to individuals and families experiencing homelessness and farmworkers and their dependents. The HCH/FH program serves individuals experiencing homelessness and farmworkers (including their dependents). On average, the program serves about 5000 unique patients a year (1200 farmworkers and 3800 homeless). The HCH/FH program ensures continuity and addresses the unique needs of this vulnerable population. By collaborating with community partners, the program continuously adapts to reduce health disparities and promote long-term health outcomes. Coastside Hope’s approach is culturally sensitive and client-centered, ensuring that services are accessible and respectful of diverse backgrounds. The organization’s bilingual and bicultural staff help to minimize language and cultural barriers, with all program materials available in both English and Spanish.
FISCAL IMPACT:
The term of the agreement is January 1, 2025, through December 31, 2027. The amount of the agreement is not to exceed $424,233 for the three-year term. Funds in the amount of $70,706 are included in the SMMC FY 2024-25 Adopted Budget. Funds in the amount of $141,411 will be included in the SMMC FY 2025-26 Recommended Budget.
The cost of the agreement is grant funded by HHS as indicated above. As a result, entering into the agreement is not expected to cause a net fiscal impact due to inclusion of those funds within SMMC’s budget.
