BACKGROUND:

Measure K is the half-cent general sales tax initially approved by San Mateo County voters in November 2012 and extended in November 2016 for a total of thirty years.

 

The Board of Supervisors (Board) held study sessions on Measure K expenditures and approved funds for the fiscal year (FY) 2025-26 budget cycle, divided equally among the five supervisorial districts, for one-time district-discretionary needs and projects. District 4 has submitted a request to use their district-discretionary Measure K funds as shown below and described in the Project Summary section of this memorandum:

This item is consistent with the criteria for district-discretionary Measure K funds approved by the Board in December 2018. District 4 indicates that this organization is either a public agency operating within the County, or a nonprofit organization registered and in good standing with the California Attorney General. District 4 indicates that the sponsoring Board member, member’s family, and member’s supervisorial staff do not serve as an officer, director, or play a policy role for the grantee organization.

 

PROJECT SUMMARY:

This is a request to authorize a grant to Community Initiatives, acting as the fiscal sponsor for BACHAC, and resulting grant agreement, in an amount not to exceed $35,000, to strengthen youth leadership development and improve culturally responsive evaluation of community-based behavioral health initiatives. The County Executive’s Office will administer and manage the proposed agreement.

 

District-discretionary Measure K funding for this organization has previously been provided as listed below: 

                     On January 28, 2025, the Board of Supervisors approved district-discretionary Measure K funding, sponsored by District 2, not to exceed $50,000, to Community Initiatives, acting as the fiscal sponsor for BACHAC, for health screenings and education through its Community Health Villages model.

                     On April 25, 2023, the Board of Supervisors approved district-discretionary Measure K funding, sponsored by District 2, not to exceed $70,000, to Community Initiatives, acting as the fiscal agent for BACHAC, for partial funding for the “Umoja Health Village,” a two-year pilot program to help low-income residents of San Mateo County to access medical services and benefits

 

BACHAC is a community-based organization dedicated to eliminating health disparities across generations and fostering diverse communities through education and services.  As noted above, BACHAC in 2025 was awarded Measure K grant funding in order to:  (1) continue the Community Health Village model to reach underserved community members directly in their neighborhoods and provide access to health screenings and education, and health care and mental health professionals,  (2) continue BACHAC’s outreach for its Community Mammograph Program and provide mammograms to over 100 individuals, including assisting uninsured individuals receiving mammograms to then qualify for health insurance coverage, (3) provide screenings for cancers, blood pressure, diabetes, HIV/STDs, and other conditions; and (4) support staffing needs, marketing materials, supplies, program incentives and equipment for community and civic events to reach underserved community members. 

 

BACHAC partnered with San Mateo County Behavioral Health and Recovery Services to lead a first-of-its-kind community assessment examining the factors that contribute to or prevent substance use disorder and mental health challenges among African American residents. The assessment engaged a diverse group of community leaders including youth, educators, faith leaders, and mental health professionals, and resulted in several key recommendations, including increased support and leadership opportunities for African American youth.  Through that work, BACHAC launched a Black Youth Advisory Board (BlackYAB, or “Youth Advisory Board”) in 2024, forming a youth advisory board focused on promoting leadership development, mentorship, and civic engagement opportunities.  The high school students serving on the Youth Advisory Board volunteer their time and efforts to identify needs within the County and to help put them into action, through workshops, community events, and public presentations which help them develop skills in communication, leadership, teamwork, and project planning, while contributing to initiatives that strengthen community wellbeing.  Recently, the Youth Advisory Board identified a need for and helped to organize a Black Student Leadership Conference and accompanying workshops. All youth residing in San Mateo County, regardless of race or ethnicity, are welcome and encouraged to attend any events hosted or facilitated by the Youth Advisory Board.

 

This district-discretionary grant will be used to fund the following project or services:

This Measure K grant will support key BACHAC program components such as mental health consultants and culturally sensitive evaluators that will strengthen youth engagement and ensure culturally responsive program implementation and evaluation, and providing leadership development opportunities for youth while strengthening community-informed approaches to addressing mental health and substance use challenges in San Mateo County.

 

The project or services funded by this district-discretionary grant will be implemented or provided from April 7, 2026, through October 31, 2027. The grantee organization will use the Measure K grant as follows:

                     Mental Health Consultants ($10,000) will provide culturally informed guidance and support for youth participants in at least two events focused on youth mental health.

                     Culturally competent evaluators ($10,000) will assist in assessing program outcomes and identifying opportunities to strengthen services for communities of color.

                     Stipends for members of the Youth Advisory Board ($15,000) to recognize the time and leadership contributions of Youth Advisory Board participants and support their continued engagement in program activities.

This district-discretionary grant will enable the organization to address an unmet service gap within the County: in culturally responsive youth leadership and mental health engagement programs for youth in San Mateo County.

 

District 4 has informed the grantee organization this grant is one-time only, and that the grantee organization will need to identify additional funding sources moving forward.

 

Total Measure K Request: Not to Exceed $35,000

 

The release of funds will be contingent on the execution of an agreement providing for the County’s confirmation of the expenditure of funds for the purposes stated herein. The County will disburse the funds to the following organization for the purposes described above:

COMMUNITY IMPACT:

Funding for this project supports the County’s Shared Vision by strengthening youth leadership, advancing health equity, and promoting community wellbeing in San Mateo County. The Bay Area Community Health Advisory Council (BACHAC), with Community Initiatives serving as its fiscal agent, will implement programming that engages youth through mentorship, leadership development, and community dialogue focused on mental health and wellbeing. By centering youth voice and leadership and supporting culturally competent program evaluation, this initiative strengthens community-informed solutions and contributes to the County’s goal of building healthy, inclusive, and resilient communities.

 

FISCAL IMPACT:

There are sufficient Measure K funds for this specific FY 2025-26 Measure K request. These funds are budgeted in the Non-Departmental Services FY 2025-26 Adopted Budget.
