Adopt a resolution authorizing an agreement with Health Plan of San Mateo to fund a comprehensive Community Health Assessment (CHA) for San Mateo County for the term of December 1, 2025 through December 31, 2026, in an amount not to exceed $340,000.

BACKGROUND:
Since 2022, San Mateo County Health has been moving towards alignment with best practices that support Community Health Assessments (CHA) and Community Health Improvement Planning (CHIP) for Local Health Jurisdictions (LHJs).
The California Department of Public Health (CDPH) released guidance indicating LHJs are responsible for CHA and CHIP planning. The guidance also specifically indicates the requirement for Managed Care Plans (MCPs) to meaningfully participate in the jurisdiction’s CHA/CHIP activities. Meaningful support is defined as data sharing and funding CHA/CHIP activities and priorities. This guidance applies to the MCPs in San Mateo County - the Health Plan of San Mateo and Kaiser Permanente.
In accordance with the new regulatory requirements from CDPH, funding requests were submitted to both MCPs. An agreement with Kaiser Permanente in the amount of $60,000 was signed January 6, 2026. Health Plan of San Mateo awarded $340,000 for San Mateo County’s next CHA, which will be completed by June 2028. This effort and subsequent analysis will ultimately inform the data community members will review to determine San Mateo County’s next set of CHIP priorities, which are slated for identification by December 2028.
DISCUSSION:
This agreement will allow San Mateo County Health to access the funding awarded from Health Plan of San Mateo that will support facilitation of the next CHA activities. Activities include selecting vendors that will support the administration of the next Health & Quality of Life Survey for quantitative data collection, as well as organizations suited to help San Mateo County Health with planning and implementation of qualitative data collection. This effort will require engaging community-based leaders and organizations that can help San Mateo County Health better understand the experiences, trends, and needs across San Mateo County. This is meant to supplement learnings extrapolated from the Health & Quality of Life Survey.
This grant agreement is going to this Board late due to the grant agreement being received shortly before its effective start date.
The resolution contains the County’s standard provisions allowing amendment of the County’s fiscal obligations by a maximum of $25,000 (in aggregate).
The agreement and resolution have been reviewed and approved by the County Attorney as to form.
It is anticipated that 3,000 residents will be engaged through the CHA process.

COMMUNITY IMPACT:
The CHA is meant to assess the health experiences, trends, and opportunities for all, with particular emphasis on engaging community members whose voices and experiences might not otherwise be represented in typical CHA processes or quantitative analysis alone. San Mateo County Health will work with Health’s Epidemiology & Evaluation team, community members, and other representatives from Health regarding what additional voices and experiences should be engaged in planning facilitation and implementation of the CHA.
FISCAL IMPACT:
The term of the agreement is December 1, 2025 through December 31, 2026. The amount of the agreement is not to exceed $340,000 for the one-year term. Funds will be included in the Public Health, Policy, and Planning FY 2026-27 Recommended Budget.
Since the County is receiving these grant funds from Health Plan of San Mateo, there is no Net County Cost associated with this agreement.
