Adopt a resolution authorizing an amendment to the agreement with Melissa Selner, DPM to expand podiatry services, increasing the amount by $125,000 to an amount not to exceed $225,000, with no change to the contract term.

BACKGROUND:
San Mateo County is responsible for providing necessary medical care to its medically indigent population. Since 2008, Dr. Selner has provided podiatry services for San Mateo Medical Center (SMMC). SMMC’s patient population has a high demand for podiatry services, which treats foot and lower leg injuries as well as ongoing health issues like diabetes that can affect lower extremities.
DISCUSSION:
Under this amendment, Dr. Selner has agreed to increase her clinical commitment from a 0.14 FTE to a 0.34 FTE; Dr. Selner will be providing services at a minimum of 168 podiatry clinics, seeing at least 12 patients per clinic, compared to 72 clinics previously. Dr. Selner’s clinical commitment is increasing due to the high demand of podiatry services and the upcoming retirement of another podiatrist at SMMC.
County Administrative Memorandum B-1 provides that contracts for physicians are exempt from the Request for Proposals process.
The amendment and resolution have been reviewed and approved by the County Attorney as to form.
The resolution contains the County’s standard provisions allowing amendment of the County fiscal obligations by a maximum of $25,000 (in aggregate).
It is anticipated that Dr. Selner will review incoming referrals within one business day for urgent cases and three business days for routine cases, 100% of the time.

FISCAL IMPACT:
The term of the amended agreement is November 1, 2021 through October 31, 2023. The amount of the amended agreement is not to exceed $225,000 for the two-year term. This amendment increases the contract amount by $125,000. Funds in the amount of $217,328 are included in the SMMC FY 2022-23 Recommended Budget.
Expenses at SMMC are covered by fees for services or third-party payors whenever possible. The portion of expenses for services provided to the medically indigent or to those covered by programs that do not meet the full costs of care is covered by the County’s General Fund contribution to SMMC and is within the existing annual appropriation.
