BACKGROUND:
Measure K is the half-cent general sales tax initially approved by San Mateo County voters in November 2012 and extended in November 2016 for a total of thirty years.
The Board of Supervisors (Board) held study sessions on Measure K expenditures and approved funds for the fiscal year (FY) 2023-24 budget cycle, divided equally among the five supervisorial districts, for one-time district-discretionary needs and projects. District 2 has submitted a request to use their district-discretionary Measure K funds as shown below and described in the Project Summary section of this memorandum:

This item is consistent with the criteria for district-discretionary Measure K funds approved by the Board in December 2018. District 2 indicates that this organization is either a public agency operating within the County, or a nonprofit organization registered and in good standing with the California Attorney General. District 2 indicates that the sponsoring Board member, member’s family, and member’s supervisorial staff do not serve as an officer, director, or play a policy role for the grantee organization.
PROJECT SUMMARY:
This is a request to authorize a grant to Community Initiatives, acting as the fiscal sponsor for the Bay Area Community Health Advisory Council, and resulting grant agreement, in an amount not to exceed $50,000, to provide for health screenings and education through its Community Health Villages model. The County Executive’s Office will administer and manage the proposed agreement.
District-discretionary Measure K funding for this organization has previously been provided as listed below:
• On April 25, 2023, the Board of Supervisors approved district-discretionary Measure K funding, in the amount of $70,000, to Community Initiatives, acting as the fiscal agent for the Bay Area Community Health Advisory Council for partial funding for the "Umoja Health Village," a two-year pilot program to help low-income residents of San Mateo County to access medical services and benefits.
The Bay Area Community Health Advisory Council (BACHAC), established in 1995, is dedicated to raising awareness of significant health issues impacting Black and diverse communities and providing healthcare services to underserved populations. Community Initiatives serves as BACHAC’s fiscal sponsor.
This district-discretionary grant will be used to fund the following project or services for residents of San Mateo County:
• Continue the Community Health Village model to reach underserved community members directly in their neighborhoods and provide access to health screenings & education, and health care and mental health professionals.
• Continue outreach for the Community Mammogram Program and provide mammograms to over 100 individuals. BACHAC will assist those uninsured individuals who receive mammograms to then qualify for health insurance coverage.
• Provide screenings for cancers, blood pressure, diabetes, HIV/STDs, and other conditions.
• Support staffing needs, marketing materials (printed flyers, handouts, content and design of social media posts), supplies, program incentives and equipment for community and civic events to reach underserved community members.
Budgeted personnel will assist the project in the following ways:
• The Executive Director will oversee the project, supervise staff, recruit partners & sponsors, and facilitate Community Health Village partners, planning committee, managing Project Coordinator.
• The Program Manager/Coordinator will supervise and coordinate volunteers and community outreach workers, assisting with canvassing and outreach events and the Community Health Village.
• Promotoras, or community outreach workers, will canvass, table at events, and provide face to face interaction and navigation as needed.
This district-discretionary grant will enable the organization to meet the following currently unmet need or service gap within the County: healthcare services and information for underserved and underinsured populations.
District 2 has informed the grantee organization that this grant is one-time only, and that the grantee organization will need to find other sources of funding going forward. The grantee organization will use the Measure K grant as follows:

The project or services funded by this district-discretionary grant will be implemented or provided during 2025 and is estimated to be completed by June 30, 2025.
The following other community organizations may be involved in or contributing funds to this project/these services, as follows:
San Mateo County Health, Gilead, Kaiser Permanente, Sierra Health Foundation and University of California, San Francisco.

The release of funds will be contingent on the execution of an agreement providing for the County’s confirmation of the expenditure of funds for the purposes stated herein. The County will disburse the funds to the following organization for the purposes described above:

EQUITY IMPACT:
Significant health disparities persist for disadvantaged communities in District 2 and San Mateo County, including limited access to health insurance, difficulty accessing healthcare providers, high rates of mental health challenges, elevated risks of cancer and high blood pressure, and disproportionate rates of asthma. The initiatives funded through this grant aim to mitigate these disparities and improve health outcomes for all residents by bringing services directly to underserved community members.
FISCAL IMPACT:
There are sufficient Measure K funds for this specific FY 2024-25 Measure K request. These funds are budgeted in the Non-Departmental Services FY 2024-25 Adopted Budget.
