Adopt a resolution authorizing an amendment to the agreement with LifeMoves to provide care coordination services, extending the term of the agreement by two years through December 31, 2027, and increasing the amount of the agreement by $450,000 to an amount not to exceed $665,000.

BACKGROUND:
San Mateo County Health has been awarded the Health Care for the Homeless/Farmworker Health (HCH/FH) grant from the Bureau of Primary Health Care, Health Services and Resources Administration of the US Department of Health and Human Services (HHS). The grant funds San Mateo Medical Center’s (SMMC) ongoing HCH/FH program, which is designed to assist homeless and farmworker adults, families, and youth with access to and utilization of SMMC’s continuum of care and other medical, health, and support services. In July 2024, the HCH/FH program completed a Request for Proposals (RFP) for case management and care coordination services.
LifeMoves was elevated for their 13+ years of experience providing case management services to individuals experiencing homelessness in San Mateo County. Their deep expertise and long-standing relationships with clients have fostered trust and led to consistently positive outcomes, such as stable housing and improved well-being. Their proven track record and commitment to tailored, impactful services make them a reliable partner in addressing homelessness in the community.
DISCUSSION:
This amendment will add transportation assistance services, extend the term of the agreement up to the three-year term as advertised in the original RFP, and increase the amount of the agreement by $450,000, to an amount not to exceed $665,000. This agreement is going to this Board late due to prolonged negotiations with the vendor.
The County Attorney has reviewed and approved the resolution and amendment as to form.
The resolution contains the County’s standard provisions allowing amendment of the County fiscal obligations by a maximum of $25,000 (in aggregate).
It is anticipated that LifeMoves will provide medical care coordination for at least 200 unique clients annually.

COMMUNITY IMPACT:
The HCH/FH program is dedicated to providing comprehensive, culturally competent, and accessible healthcare services to individuals and families experiencing homelessness and farmworkers and their dependents. The HCH/FH program serves individuals experiencing homelessness and farmworkers, including their dependents. On average, the program serves approximately 5,000 unique patients a year (1,200 farmworkers and 3,800 homeless). The HCH/FH program ensures continuity and addresses the unique needs of this vulnerable population. By collaborating with community partners, the program continuously adapts to reduce health disparities and promote long-term health outcomes.
FISCAL IMPACT:
The term of the amended agreement is January 1, 2025 through December 31, 2027. The amendment increases the amount payable under the agreement by $450,000 to an amount not to exceed $665,000 for the three-year term. Funds in the amount of $225,000 are included in the SMMC FY 2025-26 Adopted Budget. Funds in the amount of $225,000 are included in the SMMC FY 2026-27 Recommended Budget. Similar arrangements will be made for future years.
The cost of the agreement is 100% grant-funded by HHS as indicated above, with no Net County Cost as a result.
