Adopt a resolution authorizing the acceptance of an award from the California Department of Public Health Tuberculosis Control Branch (CDPH-TB) to provide tuberculosis prevention and control activities, for the term of July 1, 2024 through June 30, 2025, in an amount not to exceed $343,377.

BACKGROUND:
CDPH-TB provides funding to assist jurisdictions in performing Tuberculosis (TB) control activities. San Mateo TB Control Program provides Public Health Nurse (PHN) case management for all active and latent TB cases throughout their treatment, case and contact investigation, home and worksite investigations, Directly Observed Therapy (DOT) and Video Directly Observed Therapy (VDOT) for all cases. Additionally, CDPH-TB monitors Civil Surgeon reporting of TB screening for immigration status adjustor applicants and tracks TB screening for all immigration B Waivers. San Mateo County ranks as the sixth highest county for TB incidence in the state of California.
DISCUSSION:
The TB cases in San Mateo County illustrate extreme disparities by race, ethnicity, and place of birth. In 2022, 96% of TB cases in San Mateo County were born in Asian countries, with 41% of those being born in the Philippines. Twenty percent of cases were born in Latin American countries. One-third (33%) of TB cases in San Mateo County lived in Daly City, where 51% of the population is foreign-born. Funding from this award will be used to offset the cost of 3.0 FTE of Community Workers. The Community Workers provide DOT, VDOT, transportation to medical appointments, and daily communication between the patient and the PHN case manager. TB cases are typically followed for six to twelve months to ensure treatment is complete and they no longer have TB.
The resolution contains the County’s standard provisions allowing amendment of the County’s fiscal obligations by a maximum of $25,000 (in aggregate).
The agreement and resolution have been reviewed and approved by County Attorney as to form. This agreement is coming to the Board late due to delays with the state grantor.
It is anticipated that approximately 63 active TB cases will be case managed by County Health’s TB control program to ensure that those patients are no longer infectious, with exposed contacts investigated and managed accordingly.

EQUITY IMPACT:
This funding aims to address health disparities experienced by the large foreign-born population living in San Mateo County, who come from countries where TB is endemic. This funding pays for Community Workers who provide DOT, VDOT, transportation and daily communication between patient and PHN case manager in order to successfully complete treatment and be cleared of TB.
FISCAL IMPACT:
The term of the agreement is July 1, 2024 through June 30, 2025. The amount of the agreement is not to exceed $343,377 for the one-year term. These funds are included in the Public Health, Policy and Planning FY 2024-25 Recommended Budget. Since the County is receiving these funds and no matching funds are required, there is no Net County Cost associated with this agreement.
