Adopt a resolution authorizing an amendment to the agreement with Sonrisas Dental Health, Inc. to provide dental health services, extending the term of the agreement through June 30, 2021 and increasing the amount payable by $80,150, to an amount not to exceed $500,125.

BACKGROUND:
On February 27, 2018, your Board approved an agreement between Sonrisas Dental Health, Inc. (Sonrisas) to provide dental health services to homeless and farmworker individuals in San Mateo County as part of a grant received from the Bureau of Primary Health Care, Health Resources, and Services Administration (HRSA). The agreement has been amended multiple times, most recently to extend the term through December 31, 2020, with an associated increase in funding.
DISCUSSION:
The Health Care for the Homeless and Farmworkers Health Program (HCH/FH) had planned to release a Request for Proposal (RFP) for dental services in the second quarter of 2020. However, due to the COVID-19 pandemic, the release of the RFP has been delayed. In order to provide enough time to release an RFP as well as ensure continuity of services for the homeless and farmworker community, HCH/FH is proposing that your Board approve this amendment extending the term of the agreement through June 30, 2021.
The amendment and resolution have been reviewed and approved by County Counsel as to form.
The resolution contains the County’s standard provisions allowing amendment of the County fiscal obligations by a maximum of $25,000 (in aggregate).
The County’s Contract Compliance Committee has approved a waiver request for a forty-two months term for this agreement.
It is anticipated that the number of homeless individuals, farmworkers and their dependents presenting for dental health care services will increase to 135 individuals annually.

FISCAL IMPACT:
The term of the amended agreement is January 1, 2018 through June 30, 2021. The amount of the agreement is not to exceed $500,125. The amendment increases the contract amount by $80,150. Funds in the amount of $80,150 are included in the SMMC FY 2020-21 Adopted Budget.
Expenses for this agreement are completely funded by the ongoing Section 330 Public Health Act Community Health Center grant award from HRSA.
