Subject: Amendment to the Agreement with the City and County of San Francisco Department of Public Health to Provide HIV/AIDS Health Services
RECOMMENDATION:
title
Adopt a resolution authorizing an amendment to the agreement from the City and County of San Francisco Department of Public Health approving the annual allocation under the agreement to provide HIV/AIDS health services, for the term of March 1, 2023 through February 29, 2028, in an amount not to exceed $8,559,404.
body
BACKGROUND:
The Health Resources & Services Administration (HRSA) provides funding to eligible metropolitan areas (EMA) that are disproportionately affected by HIV/AIDS. EMAs utilize this funding for the development, organization, coordination, and operation of more effective and cost-efficient systems that deliver essential services to HIV/AIDS-affected individuals and their families. Funding comes to the Public Health, Policy and Planning (PHPP) division’s San Mateo County Public Health STD/HIV Program (SMSH) through an agreement with the San Francisco EMA’s fiscal agent, the City and County of San Francisco (CCSF).
The original agreement for the term of March 1, 2023 through February 29, 2028, in an amount not to exceed $8,559,404 was approved by this Board on November 29, 2023. CCSF is requiring a yearly agreement to allocate the specific annual amount as meted out by HRSA.
With the approval and adoption of this amendment, we are requesting that this Board also approve all future annual allocation agreements that fall under this agreement and will allow the PHPP Director or their designee to sign and approve future annual allocations under this agreement for the term of March 1, 2023 through February 29, 2028, in an amount not to exceed $8,559,404.
DISCUSSION:
SMSH utilizes Ryan White grant funds to provide a continuum of services which include outpatient ambulatory medical care, medical case management, housing assistance, outpatient and residential drug treatment, oral care, mental health, a food program, laboratory costs and medical supplies.
The resolution contains the County’s standard provisions allowing amendment of the County’s fiscal obligations by a maximum of $25,000 (in aggregate).
The agreement and resolution have been reviewed and approved by County Attorney as to form.
It is anticipated that the viral load of 90% of clients receiving services will decrease or remain undetectable as a result of the medical and psycho-social services received.

EQUITY IMPACT:
In 2021 in San Mateo County, 58.7% of people newly diagnosed with HIV were of Hispanic/Latino descent, making up more than half of new HIV infections in the same year. This funding allows staff to provide a continuum of services to this population. Such services include outpatient ambulatory medical care, medical case management, housing assistance, outpatient and residential drug treatment, oral care, mental health, a food program, laboratory costs and medical supplies.
FISCAL IMPACT:
The term of the agreement is March 1, 2023 through February 29, 2028. The amount of the agreement is not to exceed $8,559,404 for the five-year-term. Funds are included in the PHPP FY 2023-24 Recommended Budget. Similar arrangements will be made for future years. Since the County is receiving these funds, there is no Net County Cost associated with this agreement.
