BACKGROUND:
Measure K is the half-cent general sales tax initially approved by San Mateo County voters in November 2012 and extended in November 2016 for a total of thirty years.
The Board of Supervisors held study sessions on Measure K expenditures and approved funds for the fiscal year (FY) 2025-26 budget cycle, divided equally among the five supervisorial districts, for one-time district-discretionary needs and projects. District 5 has submitted a request to use a portion of its district-discretionary Measure K funds as shown below and described in the Project Summary section of this memorandum:

This item is consistent with the criteria for district-discretionary Measure K funds approved by the Board in December 2018. District 5 indicates that this organization is either a public agency operating within the County, or a nonprofit organization registered and in good standing with the California Attorney General. District 5 indicates that the sponsoring Board Member, member’s family, and member’s supervisorial staff do not serve as an officer, director, or play a policy role for the grantee organization.
PROJECT SUMMARY:
This is a request to authorize a grant to Peninsula Volunteers, Inc. (PVI), and resulting grant agreement, in an amount not to exceed $15,000, to support Quiescence, a specialized, comprehensive program for family caregivers and older adults, including those with dementia, in San Mateo County. The program promotes independence, reduces social isolation, and supports community connection through tailored, non-medical services. The County Executive’s Office will administer and manage the proposed agreement.
Aligned with the County’s ongoing commitment to aging-in-place strategies, Quiescence aligns with these efforts by providing critical support to family caregivers, reducing the emotional, financial, and psycho-social burdens of caregiving, and enabling older adults to remain in their homes with dignity and joy.
A critical component of supporting seniors and caregivers is addressing social isolation and encouraging community engagement. The Quiescence program achieves this by offering personalized assistance that connects older adults to their communities and provides caregivers with a supportive network. As a community engagement strategy, Quiescence delivers weekly in-person visits, virtual support, and detailed updates, ensuring older adults stay connected and caregivers feel supported.
Key program features include:
1. Customized Support Planning - Expert guidance on support plans, equipment, and home-based resources.
2. Service Coordination - Streamlined arrangements for medical, non-medical, and community support services.
3. Household Management - Connection to vetted providers for repairs, housekeeping, and maintenance.
4. Specialized Provider Coordination - Referrals to physical therapists, home health aides, and other specialists.
5. Transportation Support - Assistance with transportation to appointments and community activities.
6. Community Connection - Weekly in-person visits for wellness checks, friendly visits, and engagement.
7. Caregiver Support - Virtual support Monday-Friday plus detailed weekly status reports.

As a community-focused program, Quiescence relies on local funds and grants to sustain and expand its services. This grant of up to $15,000 will enable PVI to provide subsidized support to families in San Mateo County who are unable to afford the program, ensuring equitable access to services that reduce isolation, empower caregivers, and promote a vibrant, connected community for older adults.
District-discretionary Measure K funding for this organization has previously been provided as listed below:
• 5/8/2018, County Manager: Measure K: Adopt a resolution authorizing a one-time grant of district-specific Measure K funds, not to exceed $10,000, to Peninsula Volunteers Inc. for expanding transportation services to seniors, and authorizing the County Manager, or his designee, to prepare and execute the grant agreement.
• 4/9/2019, District 4: Measure K: Adopt a resolution authorizing a one-time grant of district-discretionary Measure K funds, not to exceed $40,000 to Peninsula Volunteers, Inc. for two Ford Transit Connect vans to be used for Meals on Wheels deliveries, and authorizing the County Manager, or his designee, to execute the grant agreement.
This district-discretionary grant will enable the organization to meet the following currently unmet need or service gap within the County: There is a gap in affordable, non-medical in-home support for low-income older adults in the County who wish to age in place. Many face challenges like isolation, difficulty managing daily tasks, and lack of transportation, with limited programs available to help. This grant will allow Peninsula Volunteers, Inc. (PVI) to provide these essential services at no cost to individuals who would otherwise go without support.
District 5 has informed the grantee organization that this grant is one-time only, and that the grantee organization will need to find other sources of funding going forward. The grantee organization will use the Measure K grant as follows:
With the $15,000 Measure K grant, Peninsula Volunteers, Inc. (PVI) will serve at least 5 individuals who are unable to afford the Quiescence program’s services, providing each with comprehensive support for a 3-month period. At an average cost of $1,000 per individual per month, this measure ensures that 5 individuals receive tailored, non-medical support, including customized support planning, service coordination, household management, transportation support, and weekly in-person visits to address needs and foster connection, totaling approximately $15,000 in program costs.

The release of funds will be contingent on the execution of an agreement providing for the County’s confirmation of the expenditure of funds for the purposes stated herein.
