BACKGROUND:

The County offers an award-winning Wellness Program as part of its employee Wellness Policy, a key component of its strategy to be a competitive employer that attracts and retains a skilled and productive workforce. The Wellness Program offers a diverse range of programs, services, and activities including biometric screenings, health education classes, and mental health resources. 

 

In 2017, the County conducted its first request for proposals (RFP) to select a vendor to provide wellness program services. The County selected Zomo Health, and the Board authorized agreements from September 2018 through December 2022. (Resolution Nos. 076168 and 078629). Zomo Health offers a wellness portal, PreventionCloud, that facilitates the County’s Wellness Dividend Program, Online Health Assessment, event registration, biometric screenings, social connections, emotional wellbeing resources, physical activity and mindfulness challenges, Medical Plan-specific resources and on-demand wellness videos. Zomo Health also administers and reports on the Dividend Program and Health Risk Assessment, manages vendor contracts, creates on-demand marketing materials, and offers unlimited employee access to Health Coaches, a Wellness Nurse, a Registered Dietician, and heath education classes. 

 

In 2022, the Human Resources Department conducted another RFP for these wellness engagement portal and support services to ensure that the County continued to receive the best value for these services. At that time, Zomo Health was the only RFP applicant capable of serving the breadth of the County’s whole-person wellbeing program. The Board authorized an agreement with Zomo Health for an amount not to exceed $2,090,850 and a term of January 1, 2023 – December 31, 2025. (Resolution No. 079380).

 

 

DISCUSSION:

In 2024, the County extended the Dividend Program from only employees on County health plans to all Regular and Extra Help employees. This drew in thousands more employees to participate in wellness program activities.

 

During this time, the wellness portal—accessible by website and mobile app—has achieved unprecedented employee engagement, with 77% of employees having logged in, 71% having completed the Health Risk Assessment, and 65% having earned the Wellness Dividend. The portal is available to both employees and spouses, is easy to navigate, HIPAA compliant, and incorporates important reminders and legal waivers.

 

Zomo Health has continued to customize the County’s wellness portal based on Department input, County health plan feedback, and Health Risk Assessment results.  Zomo Health’s customized reporting and analytics on employee engagement and health help the County make informed decisions about the Wellness Program to support employee health. Zomo Health also supports the County’s Wellness Committee through demonstrations, trainings, site visits, and custom Wellness Champion accounts.

 

In addition, since 2022, the County has expanded wellness services facilitated by Zomo Health, including adding departmental support services, increasing on-site presence (including visiting 40+ County sites), extending outreach to under-engaged employee groups, and creating personalized “My Plans” for employees to improve biometrics and health outcomes. 2024 wellness survey results indicate that increased on-site support has increased employee participation in program activities. These extended services have required additional staffing support from Zomo Health.

 

The Human Resources Department would like to extend the agreement with Zomo Health for one additional year—for a total four-year contract—and increase the not-to-exceed amount by $1,409,150 to cover the expanded services and term. This will allow for increased on-site support for the remainder of 2025 and 2026 to bolster departmental efforts related to employee wellbeing and retention, expand capacity for wellness events, increase training of departmental Wellness Champions, increase the volume of Wellness Activities, expand the annual Wellness Fair, restart on-site health coaching, and better serve smaller work locations like North County and Coastside.

 

The County will conduct another Request for Proposals (RFP) for these wellness services in spring 2026.

 

The accompanying resolution contains the County’s standard provision allowing amendments of the County’s fiscal obligations by a maximum of $25,000 (in aggregate).

FISCAL IMPACT:

The County’s cost for the Zomo Health agreement is covered by the Benefits Trust Fund. These costs are included in the medical insurance rates and are shared between the County and employees. This amendment will increase the total current contract from $2,090,850 to $3,500,000, as a result of both the increased term and the expanded services.
