Adopt a resolution authorizing an agreement with Renata Jarosz, DO to provide physical medicine and rehabilitation services for the term of October 1, 2020 through September 30, 2022, in an amount not to exceed $680,000.

BACKGROUND:
Since September 2014, Dr. Renata Jarosz has provided musculoskeletal clinical and physical medicine (physiatry) services for San Mateo Medical Center (SMMC). The main purpose of these services is to diagnose and treat patients with complex musculoskeletal pain syndromes and to triage referrals to orthopedic surgery pain management and neurosurgery services. These services ensure that only surgical patients are referred to orthopedics and neurosurgery, and physiatry patients are treated non-surgically. Because patients are now appropriately sorted, patients in both the surgical and non-surgical settings are seen more quickly.
DISCUSSION:
Under the terms of the agreement, Dr. Jarosz will continue to provide physiatry services five days each week to SMMC patients. In addition, Dr. Jarosz also will serve as Medical Director of Physiatry and Rehabilitation Medicine, as well as Medical Director for the Physical and Occupational Therapy Departments.
County Administrative Memorandum B-1 provides that contracts for physicians are exempt from the Request for Proposals process.
The agreement and resolution have been reviewed and approved by County Counsel as to form.
The resolution contains the County’s standard provisions allowing amendment of the County fiscal obligations by a maximum of $25,000 (in aggregate).
It is anticipated that Dr. Jarosz will complete a projected volume of 4,421 Relative Value Units (RVUs) per annum. RVUs are the basic component of the Resource-Based Relative Value Scale (RBRVS), which is a methodology used by the Centers for Medicare & Medicaid Services (CMS) and private payers to determine physician payment. RVUs define the value of a service or procedure relative to all services and procedures. This measure of value is based on the extent of physician work, clinical and nonclinical resources, and expertise required to deliver the healthcare service to patients.

FISCAL IMPACT:
The term of the agreement is October 1, 2020 through September 30, 2022. The amount of the agreement is not to exceed $680,000 for the two-year term. Funds in the amount of $340,000 are included in the SMMC FY 2020-21 Recommended Budget. Funds in the amount of $340,000 will be included in the SMMC FY 2021-22 Recommended Budget.
The payment provisions for this contract are the same as the previous contract, as per the Medical Group Management Association 2019 total compensation rate, a tool SMMC uses to benchmark provider rates.
Expenses at SMMC are covered by fees for services or third-party payors whenever possible. The portion of expenses for services provided to the medically indigent or to those covered by programs that do not meet the full costs of care is covered by the County’s General Fund contribution to SMMC, and is within the existing annual appropriation.
