Subject:                      Amendment to the Agreement with Crystal Chen, OD to Provide Optometry Services

RECOMMENDATION:

title

Adopt a resolution authorizing an amendment to the agreement with Crystal Chen, OD to provide optometry services, increasing the amount by $5,000, to an amount not to exceed $105,000.

body

BACKGROUND:

On August 2, 2018, Dr. Chen entered into an agreement with San Mateo Medical Center (SMMC) to provide optometry services for the term of December 1, 2018 through November 30, 2019, in an amount not to exceed $100,000.

DISCUSSION:

Dr. Chen has provided more clinic days to SMMC than originally projected in the initial agreement. In addition, Dr. Chen has been providing support to the Fair Oaks Health Center model cell project. As a result, additional funds are needed to pay for these services through the end of the agreement.

This amendment is coming to the Board late due to the increase in services provided late in the term of the agreement.

The amendment and resolution have been reviewed and approved by County Counsel as to form.

It is anticipated that 100% of eye examinations will adhere to infection control standards as defined by the Joint Commission and Title XXII of the State of California, Code of Regulations.

FISCAL IMPACT:

The term of the amended agreement is December 1, 2018 through November 30, 2019. The amount of the agreement is not to exceed $105,000. The amendment increases the contract amount by $5,000. Funds in the amount of $5,000 are included in the SMMC FY 2019-20 Adopted Budget.

Expenses at SMMC are covered by fees for services or third-party payors whenever possible. The portion of expenses for services provided to the medically indigent or to those covered by programs that do not meet the full costs of care is covered by the County’s General Fund contribution to SMMC and is within the existing annual appropriation.

The resolution contains the County’s standard provisions allowing amendment of the County fiscal obligations by a maximum of $25,000 (in aggregate).
