BACKGROUND:

San Mateo County Health has been awarded the Health Care for the Homeless/Farmworker Health (HCH/FH) grant from the Bureau of Primary Health Care, Health Services and Resources Administration of the US Department of Health and Human Services. The grant funds San Mateo Medical Center’s (SMMC) ongoing HCH/FH program, which is designed to assist homeless and farmworker adults, families, and youth with access to and utilization of SMMC’s continuum of care and other medical, health and support services.

DISCUSSION:

In January 2021, the HCH/FH program completed a Request for Proposals (RFP) for enabling services. Enabling services are non-clinical services that do not include direct patient services, which enable individuals to access health care and improve health outcomes. Examples of enabling services include: case management, referrals, translation/interpretation, transportation, eligibility assistance, health education, environmental health risk reduction, health literacy, and outreach. Puente de la Costa Sur (Puente) was chosen for their efficiency in delivering enabling services to the population they serve.

 

County Counsel has reviewed and approved the resolution and agreement as to form.

 

The resolution contains the County’s standard provisions allowing amendment of the County fiscal obligations by a maximum of $25,000 (in aggregate).

 

The County’s Contract Compliance Committee has approved a waiver request for a forty-two-months term for this agreement.

It is anticipated that a minimum of 200 unduplicated farmworkers or dependents will receive care coordination services or assistance with applying for or renewing their health insurance coverage annually.

FISCAL IMPACT:

The term of the agreement is July 1, 2021 through December 31, 2024. The maximum amount to be expended under the agreement is not to exceed $574,906 for the forty-two-months term. Funds in the amount of $164,259 are included in the SMMC FY 2021-22 Adopted Budget. Similar arrangements will be made for future years.

 

The payment provisions and levels of services in this instant agreement differ from those in the previous agreement between the County and Puente, due to a reduction in the number of clients proposed to be served under this new agreement. The total amount authorized to be expended under this new agreement contract is 17% less than the amount authorized under the prior agreement.

 

Expenses at SMMC are covered by fees for services or third-party payors whenever possible. The portion of expenses for services provided to the medically indigent or to those covered by programs that do not meet the full costs of care is covered by the County’s General Fund contribution to SMMC and is within the existing annual appropriation.
