Adopt a resolution authorizing an agreement with John McNamara, DO to provide ophthalmology services for the term of July 1, 2021 through June 30, 2024, in an amount not to exceed $450,000.

BACKGROUND:
San Mateo County is responsible for providing necessary medical care to its medically indigent population, as well as access to services to its general population. Dr. McNamara has been providing ophthalmology services at San Mateo Medical Center (SMMC) for more than twenty years.
DISCUSSION:
The County’s need for ophthalmology services remains substantial due to the aging of its patient population. Dr. McNamara has agreed to provide the same level of ophthalmology services as his previous agreement and will continue to share call coverage with the other two ophthalmologists under contract with SMMC. In addition, Dr. McNamara will serve as Medical Director of Ophthalmology, with responsibility for preparing and managing the monthly call schedule and take appropriate steps to ensure manageable patient wait times for services.
County Administrative Memorandum B-1 provides that contracts for physicians are exempt from the Request for Proposals process.
The agreement and resolution have been reviewed and approved by County Counsel as to form.
The resolution contains the County’s standard provisions allowing amendment of the County fiscal obligations by a maximum of $25,000 (in aggregate).
It is anticipated that 85% of diabetic patients will receive annual retinal/dilated eye exam.

FISCAL IMPACT:
The term of the agreement is July 1, 2021 through June 30, 2024. The amount of the agreement is not to exceed $450,000 for the three-year term. Funds in the amount of $150,000 are included in the SMMC FY 2020-21 Recommended Budget. Similar arrangements will be made for future years of the term.
The payment provisions of this contract are based on the Median Ophthalmology 2019 Medical Group Management Association Provider Compensation and Production Report, which SMMC uses to benchmark its contracted provider rates.
Expenses at SMMC are covered by fees for services or third-party payors whenever possible. The portion of expenses for services provided to the medically indigent or to those covered by programs that do not meet the full costs of care is covered by the County’s General Fund contribution to SMMC and is within the existing annual appropriation.
