Adopt a resolution authorizing an agreement with Samaritan House to provide care coordination services for homeless individuals for the term of January 1, 2018, through December 31, 2020, in an amount not to exceed $243,000.

BACKGROUND:
The San Mateo County Health System has been awarded the Health Care for the Homeless/ Farmworker Health (HCH/FH) grant from the Bureau of Primary Health Care, Health Services and Resources Administration of the US Department of Health and Human Services. The grant funds San Mateo Medical Center’s (SMMC) ongoing HCH/FH program, which is designed to assist homeless and farmworker adults, families, and youth with access to and utilization of SMMC’s continuum of care and other medical, health and support services. HCH/FH has contracted with Samaritan House since 2014, to provide enabling services for the homeless.
DISCUSSION:
In September 2017, HCH/FH issued a Request for Proposals to providers to deliver health and health related services to the homeless and farmworker populations of San Mateo County. Samaritan House was chosen by the HCH/FH Program Co-Applicant Board for its innovative approach to providing care coordination/enabling services for the homeless population in the San Mateo County. These services include outreach, patient and community education, transportation, follow-up, translation services, referral services and ongoing support to improve client access to San Mateo County Health System’s primary medical services and HCH/FH Program contractors.
The agreement and resolution have been reviewed and approved by County Counsel as to form. The agreement is coming to your Board late due to protracted negotiations with the contractor.
The resolution contains the County’s standard provisions allowing amendment of the County fiscal obligations by a maximum of $25,000 (in aggregate).
The agreement contributes to the Shared Vision 2025 outcome of a Healthy Community by providing health care services to homeless individuals residing at shelters, unsheltered homeless individuals residing on the streets, and farmworker individuals of San Mateo County. It is anticipated that a minimum of 210 unduplicated homeless individuals will receive care coordination.

FISCAL IMPACT:
The term of the agreement is January 1, 2018 through December 31, 2020. The amount of the agreement is not to exceed $243,000 for the three year term. Funds in the amount of $40,500 are included in the SMMC FY 2017-18 Adopted Budget. Funds in the amount of $81,000 are included in the SMMC FY 2018-19 Recommended Budget. Similar arrangements will be made for future years.
Compared to the previous agreement, this agreement represents a 27% increase due to: a 20% increase in the number of homeless individuals to be served; general cost increases, and the inclusion of funding for trainings/conferences that was previously being supported outside of the contract.
This agreement is 100% funded by a grant awarded by the Bureau of Primary Health Care, Health Services and Resources Administration of the US Department of Health and Human Services, to fund the Health Care for the Homeless program.
