BACKGROUND:

In 2018 Behavioral Health and Recovery Services (BHRS) identified that there was a need to fill a service gap for Severely Mentally Ill adult clients that are diagnosed with moderate to severe eating disorders, as defined in the DSM-5, that require a more specialized and intensive level of treatment. At the time, no BHRS clinic or program effectively addressed the comprehensive behavioral, dietary, and medical needs associated with moderate/severe eating disorders. BHRS reached out to several local providers and Cielo House was one of two providers that met the requirements to provide comprehensive and specialized treatment for clients diagnosed with eating disorders such as: anorexia, bulimia, binge eating disorder, and eating disorders not otherwise specified. 

 

In July 2018, BHRS conducted an open and fair process to select a provider through obtaining three quotes. Cielo House submitted a quote and BHRS entered into an agreement with Cielo House, which has been providing these services since then.  Residential treatment services are exempt from the Request for Proposal process under the B-1 Administrative Memorandum.

 

DISCUSSION:

Cielo House has been providing Residential Treatment and Partial Hospitalization services to adult clients in the County. Services include nutritional assessments, comprehensive treatment plans, close supervision, dietary counseling and individual and family counseling. Due to identification of additional clients that are in need of eating disorder services, an amendment is needed at this time to cover the increased referrals.

 

The Resolution contains the County’s standard provision allowing amendment of the County’s fiscal obligations by a maximum of $25,000 (in aggregate).

 

The agreement and Resolution have been reviewed and approved by County Counsel as to form.

 

Approval of this amendment contributes to the Shared Vision 2025 outcome of a Healthy Community by providing comprehensive treatment services for Severely Mentally Ill clients diagnosed with eating disorders. It is anticipated that 50% of clients receiving services will not require hospitalization and no more than one Psychiatric Emergency Services (PES) visit.

FISCAL IMPACT:

The term of the revised agreement is July 26, 2018 through June 30, 2020. The amount of the agreement is not to exceed $300,000. This amendment increases the contract amount by $200,000. Funds in the amount of $150,000 are included in the BHRS FY 2018-19 Adopted Budged. Similar arrangements will be made in future years. Of the FY 2018-19 increase, it is anticipated that $150,000 will be funded by 1991 Realignment funds. There is No Net Cost.
