BACKGROUND:

The California Department of Health Care Services is transforming Medi-Cal through CalAIM initiatives to build a more coordinated, person-centered, and equitable health system. As part of that transformation, the Health Plan of San Mateo (HPSM) contracts with County Health’s Public Health, Policy & Planning to provide Enhanced Care Management services to medically at-risk persons experiencing homelessness, mental health challenges, and substance use disorder through the Bridges to Wellness program.

 

DISCUSSION:

HPSM seeks to grant Bridges to Wellness, through the CalAIM Incentive Payment Program, monies to create sustainable infrastructure and to expand services to the justice-involved population. Funds will be used for two limited term positions to support the Enhanced Care Management populations. Positions are intended to build technical capacity, develop best practices in alignment with state guidance and assess the need of the justice-involved population. Given the timeline of negotiations with HPSM, funds will offset costs of services that are underway. The ultimate goal is to integrate and improve care coordination within the Bridges to Wellness program.

 

This Board is being asked to authorize the Chief of San Mateo County Health or designee to execute subsequent amendments that do not exceed an aggregate of $25,000.

 

This agreement is going to this Board late due to prolonged discussions with HPSM.

The amendment and resolution have been reviewed and approved by the County Attorney as to form.

 

It is anticipated that 30 individuals will receive Enhanced Care Management in FY 2024-25 and 60 individuals in FY 2025-26.

EQUITY IMPACT:

This agreement seeks to enhance existing services to specific populations, including adults with health risks who experience homelessness, mental health challenges, and/or substance use disorder. Additionally, these funds will enable the Bridges to Wellness program to expand services to the justice-involved population. Nearly all persons receiving Enhanced Care Management live at the acutely low or extremely low-income level. Poverty, homelessness, and illness place these individuals among the most vulnerable in our county. These funds will be used to enhance and expand services to vulnerable populations to create opportunities for healthier, better lives.

 

FISCAL IMPACT:

The term of the agreement is September 1, 2024 through December 31, 2027. The amount of the agreement is not to exceed $886,056. Funds in the amount of $295,352 are included in the PHPP FY 2024-25 Adopted Budget. Similar arrangements will be made in future years. This contract is fully funded by the HPSM. There is no Net County Cost.
