Adopt a resolution authorizing an amendment to the agreement with Carol Tabak, MD to provide palliative care, general surgery, and telemedicine services, extending the term of the agreement through August 31, 2020 and increasing the amount by $35,000, to an amount not to exceed $180,000.

BACKGROUND:
On April 9, 2019, the Board approved an agreement with Dr. Tabak to provide general surgery and palliative care services.
DISCUSSION:
This amendment will add telemedicine services, in response to the shelter in place order to comply with the COVID-19 response.
The agreement and resolution have been reviewed and approved by County Counsel as to form.
The resolution contains the County’s standard provisions allowing amendment of the County fiscal obligations by a maximum of $25,000 (in aggregate).
It is anticipated that 50% of palliative and hospice related cases will be performed by using telemedicine.

FISCAL IMPACT:
The term of the amended agreement is June 1, 2019 through August 31, 2020. The amount of the agreement is not to exceed $180,000. The amendment increases the contract amount by $35,000. Funds in the amount of $11,666 are included in the SMMC FY 2019-20 Adopted Budget. Funds in the amount of $23,334 will be included in the SMMC FY 2020-21 Recommended Budget.
Expenses at SMMC are covered by fees for services or third-party payors whenever possible. The portion of expenses for services provided to the medically indigent or to those covered by programs that do not meet the full costs of care is covered by the County’s General Fund contribution to SMMC, and is within the existing annual appropriation.
