BACKGROUND:

The California Department of Public Health (CDPH) provides funding for tuberculosis (TB) control activities including the investigation of potential cases and administration of the directly-observed therapy program. San Mateo County and counties throughout the state of California continue to be challenged by: 1) the increased TB transmission rate among homeless populations in congregate settings such as: shelters, prisons and jails, 2) high immigration rates from TB-endemic countries to San Mateo communities, and 3) a general reduction in the national public health infrastructure supporting TB control activities. 

 

DISCUSSION:

TB cases and their contacts are typically followed for six to twelve months, sometimes longer. Funding from this award will be used to offset the cost of paying 1.00 full-time employee (FTE) to work as a Communicable Disease Investigator, 2.25 FTEs working as Community Workers, and .33 FTE working as a Social Work Supervisor. The TB staff will provide prevention and control measures including case management, contact investigation, and directly-observed therapy.

 

Delays in federal funding authorization prevented CDPH from sending the agreement until late June 2018.

 

The Resolution contains the County’s standard provisions allowing amendment of the County’s fiscal obligations by a maximum of $25,000 (in aggregate).

 

The agreement and Resolution have been reviewed and approved by County Counsel as to form.

 

The acceptance of the award contributes to the Shared Vision 2025 outcome of a Healthy Community by providing funds to identify and case-manage patients with TB to prevent the spread of disease. It is anticipated that approximately 70 active TB cases will be case-managed by the TB Control Program, with exposed contacts investigated and managed accordingly.

FISCAL IMPACT:

The term of the agreement is July 1, 2018 through June 30, 2019. The amount of the agreement is not to exceed $339,591 for the contract term (not to exceed $320,523 for the base award and $19,068 for food and shelter incentives respectively). These funds are included in the Public Health, Policy and Planning FY 2018-19 Adopted Budget.  Since the County is receiving these funds and no matching funds are required, there is no Net County Cost associated with this agreement.
