Adopt a resolution authorizing an agreement with Ayudando Latinos a Soñar to provide care coordination services for the term of January 1, 2025, through December 31, 2027, in an amount not to exceed $602,271.

BACKGROUND:
San Mateo County Health has been awarded the Health Care for the Homeless/Farmworker Health (HCH/FH) grant from the Bureau of Primary Health Care, Health Services and Resources Administration of the US Department of Health and Human Services (HHS). The grant funds San Mateo Medical Center’s (SMMC) ongoing HCH/FH program, which is designed to assist homeless and farmworker adults, families, and youth with access to and utilization of SMMC’s continuum of care and other medical, health, and support services. In July 2024, the HCH/FH program completed a Request for Proposals for case management and care coordination services.
DISCUSSION:
HCH/FH requested proposals for a range of case management and enabling services for homeless individuals, farmworkers, and their dependents. These services include 1. street medicine case management and care coordination, 2. intimate partner violence, domestic violence, and human trafficking case management, 3. medical care coordination for newly housed individuals, 4. promotores de salud, 5. MyChart orientation, 6. health insurance assistance, 7. transportation, and 8. nutrition education. Ayudando Latinos a Soñar (ALAS) was elevated due to their demonstrated capacity, extensive community ties, and culturally competent approach, all of which make them uniquely qualified to meet the health and welfare needs of San Mateo’s farmworker community and homeless populations effectively and compassionately. ALAS has agreed to provide promotores, care coordination, health insurance assistance, and transportation assistance services for farmworkers and their dependents for the term of January 1, 2025, through December 31, 2027, in an amount not to exceed $602,271.
County Attorney has reviewed and approved the resolution and agreement as to form.
The resolution contains the County’s standard provisions allowing amendment of the County fiscal obligations by a maximum of $25,000 (in aggregate).
It is anticipated that ALAS will provide navigation assistance to 200 unique farmworkers and their dependents per year.

EQUITY IMPACT:
The HCH/FH program is dedicated to providing comprehensive, culturally competent, and accessible healthcare services to individuals and families experiencing homelessness and farmworkers and their dependents. The HCHFH program serves individuals experiencing homelessness and farmworkers (including their dependents). On average, the program serves about 5000 unique patients a year (1200 farmworkers and 3800 homeless). The HCH/FH program ensures continuity and addresses the unique needs of these vulnerable populations. By collaborating with community partners, the program continuously adapts to reduce health disparities and promote long-term health outcomes. ALAS’s care coordination services will address community need.
FISCAL IMPACT:
The term of the agreement is January 1, 2025, through December 31, 2027. The amount of the agreement is not to exceed $602,271 for the three-year term. Funds in the amount of $100,379 are included in the SMMC FY 2024-25 Adopted Budget. Funds in the amount of $200,757 will be included in the SMMC FY 2025-26 Recommended Budget.
The cost of the agreement is grant-funded by HHS as indicated above. As a result, entering into the agreement is not expected to cause a net fiscal impact due to inclusion of those funds within SMMC’s budget.
