B)                     The San Mateo County Health Chief or the Chief’s designee to execute Older Americans Act community provider agreements, during or for the term of July 1, 2025 through June 30, 2026, in a total amount not to exceed $5,561,829.

BACKGROUND:

Every four years, the Board of Supervisors, acting as the Governing Board for the Area Agency on Aging (AAA) for the County of San Mateo, reviews and approves an Area Plan for the County’s submission to the California Department of Aging (CDA). The Area Plan provides a profile of the County’s older adult population, identifies goals for addressing the needs of older adults and adults with disabilities, and recommends that services be funded with San Mateo County’s Older Americans Act of 1965 (OAA) funds.

 

This Board approved the 2025-2026 Area Plan update on May 20, 2025.

 

In February 2022, Aging and Disability Services (ADS) issued a Request for Proposals (RFP) for the provision of OAA services to older adults and adults with disabilities for the term of July 1, 2022, through June 30, 2026. The RFP Evaluation Committee selected 21 community organizations to provide Title III and VII OAA services such as Congregate Nutrition, Home Delivered Meals, Family Caregiver Support, Long-Term Care Ombudsman services, Health Insurance Counseling and Advocacy Program (HICAP), and Medicare Improvements for Patients and Providers Act (MIPPA, which increases health insurance access for residents with low-income and/or limited English proficiency).

 

In May 2024, the Board of Supervisors, acting as the Governing Board of our AAA, authorized ADS to execute an agreement (MOU #AAA-2425-08) with CDA for the term of July 1, 2024, through June 30, 2029. The agreement authorized ADS to administer the OAA programs and accept future funding from CDA to support Area Plan goals. Funds are made available to the County through an annual Funding Request Memo CDA sends to the AAAs.

 

DISCUSSION:

OAA funding allows ADS to contract with community agencies to implement programs that achieve the Area Plan’s objectives. These programs include adult day care, adult day health care, congregate nutrition, elder abuse prevention, elder abuse education and training, family caregiver support, health promotion, home-delivered meals, legal assistance, nutrition counseling, nutrition education, the long-term care ombudsman program, information and assistance (which steers clients to appropriate community resources and supports older adults in disaster planning and preparedness), and transportation services. These programs have served an average of 29,633 clients annually, helping at-risk individuals remain in the most independent setting possible. This funding agreement with CDA allows the County to accept OAA funds to support these programs.

 

The terms of the agreements with OAA community providers are consistent with prior agreements and comply with CDA requirements. The agreements focus on serving low-income and minority individuals, older individuals with limited English proficiency, and older individuals residing in rural areas.

 

ADS requests that this Board authorize the Chief of San Mateo County Health, or the Chief’s designee, to accept the Fiscal Year 2025-26 OAA funds and to prepare and execute OAA community provider agreements for the term of July 1, 2025, through June 30, 2026. This authorization also includes the ability to approve future amendments to these agreements, including adjustments to the County’s maximum fiscal obligation, modifications to the contract term, and changes to the services provided. Any amendments will remain within the current or revised fiscal provisions, including any future one-time-only funds or additional funds received and allocated for the contract period.

 

The funding terms for the HICAP and MIPPA programs do not match those of the other OAA programs’ providers due to differences in CDA’s funding periods. The funding for HICAP is not included in the $5,561,829 and will be presented to this Board separately. The MIPPA agreement is for less than $200,000 and need not be presented to this Board.

 

The amounts of each agreement will be determined based on the funding agreement(s) released by CDA as well as County general funds allocated based on the County budget process. CDA’s initial funding allocations for Fiscal Year 2025-26 were received on May 16, 2025.

It is anticipated that OAA-funded programs will serve 25,000 unduplicated older adults in Fiscal Year 2025-26.

COMMUNITY IMPACT:

CDA allocates Area Plan funds annually to support the County’s work to improve access to services, promote aging in place, strengthen family caregiver support, and reduce social isolation for residents 60 and older. The Area Plan will positively impact older adults by helping them age in the place of their choosing. Based on 2025 demographic data from CDA, 199,329 adults 60 or older live in San Mateo County: 64% identify as racial minorities (30% as Asian, 25% as Hispanic or Latino, 6% as Other, 2% as Black or African American, and 1% as Native Hawaiian and Other Asian Pacific Islander), 8% are low income, 15% are Medi-Cal eligible, and 16% live alone.

 

FISCAL IMPACT:

The term of the OAA provider agreements is from July 1, 2025, through June 30, 2026. The total amount of the agreements is not to exceed $5,561,829 for the 12-month term. Approximately $4,640,737 is expected to come from State and Federal funds, and the remaining $921,092 is from the County General Fund. Funds for these services will be included in the ADS FY 2025-26 Recommended Budget.
