Adopt a resolution authorizing an agreement with David Shields, MD to provide gastroenterology services for the term of September 1, 2020 through August 31, 2021, in an amount not to exceed $450,000.

BACKGROUND:
San Mateo County is responsible for providing necessary medical care to its medically indigent population.
DISCUSSION:
Dr. Shields has agreed to continue providing gastroenterology (GI) services. However, Dr. Shields has agreed to provide more services than his previous agreement and will now provide a minimum of 6 half day clinic blocks per week, performing endoscopic retrograde cholangio pancreatography procedures. Dr. Shields will also provide 8 periods of weekday call, 1 period of weekend call per month, and 2 periods of holiday weekend call per year, while continuing to be medical director.
County Administrative Memorandum B-1 provides that contracts for physicians are exempt from the Request for Proposals process.
The agreement and resolution have been reviewed and approved by County Counsel as to form. This agreement is coming to your Board late due to prolonged negotiations.
The resolution contains the County’s standard provisions allowing amendment of the County fiscal obligations by a maximum of $25,000 (in aggregate).
It is anticipated that the current GI waitlist for SMMC will be reduced by 10%.

FISCAL IMPACT:
The term of the agreement is September 1, 2020 through August 31, 2021. The amount of the agreement is not to exceed $450,000 for the one-year term. Funds in the amount of $450,000 are included in the SMMC FY 2020-21 Recommended Budget.
The payment provisions have remained the same from the prior agreement to be consistent with Medical Group Management Association rates, which SMMC uses to benchmark physician contract compensation.
Expenses at SMMC are covered by fees for services or third-party payors whenever possible. The portion of expenses for services provided to the medically indigent or to those covered by programs that do not meet the full costs of care is covered by the County’s General Fund contribution to SMMC and is within the existing annual appropriation.
