Adopt a resolution authorizing a third amendment to the agreement with InterOperability Bidco, Inc. dba Rhapsody to provide Enterprise Master Person Index Software and support services within San Mateo County Health, extending the 10-year term by an additional two years through June 15, 2027, and increasing the County’s fiscal obligation by $65,814 to an amount not to exceed $1,149,206.

BACKGROUND:
San Mateo County Health (SMCH) historically had multiple electronic health record (EHR) systems that did not maintain a unified and accurate demographic profile of our patients and clients. SMCH and InterOperability Bidco, Inc. dba Rhapsody (Rhapsody) entered into an agreement to provide a single record for our patients and clients.
In November 2024, SMCH implemented a new EHR system that will ultimately centralize all our patients’ and clients’ information into one system once all divisions are incorporated into the new system.
DISCUSSION:
Rhapsody’s Enterprise Master Person Index (EMPI) is currently providing the solution to maintain a single best demographic record for each patient/client in our legacy EHR systems and Epic to ensure one record. As Epic goes live with other divisions, this will reduce the need for the Rhapsody EMPI legacy solution; thus, SMCH is planning to sunset the latter application within the next two years.
The resolution contains the County’s standard provisions allowing amendment of the County’s fiscal obligations by a maximum of $25,000 (in aggregate).
ISD has reviewed and approved the IT component of this agreement.
The County’s Contract Compliance Committee has approved a waiver request to extend the agreement an additional two years.
The agreement and resolution have been reviewed and approved by the County Attorney as to form.
It is anticipated that SMCH will resolve and maintain at least 98% of all duplicate patient registration records.

COMMUNITY Impact:
The EMPI system enhances the department's ability to consolidate patient information across diverse healthcare settings, eliminating barriers that disproportionately affect underserved populations. Through improved patient identification, data accuracy, and integration, County Health reduces health disparities and provides a foundation for informed decision-making that benefits the entire community. This supports our goal of maintaining an equitable, efficient, and compassionate healthcare system where every individual is recognized, respected, and receives the care they deserve, thus reducing disparities, improving outcomes, and fostering trust in our healthcare system. The EMPI will positively impact equitable health outcomes by enhancing the safety of services provided to the community, especially at San Mateo Medical Center (SMMC). Demographically, within the past 18 months, the largest cohorts of specialty care patients at SMMC were female (52%), Hispanic or Latino (63%), and primarily Spanish-speaking (55%). SMMC strives to eliminate bias in the medical profession by providing culturally competent training to its physicians and by following up with patients to ensure that they continue to have access to specialized care.
FISCAL IMPACT:
The term of this amended agreement is June 16, 2015 through June 15, 2027. The agreement amount is not to exceed $1,149,206. Funds in the amount of $1,306 are included in County Health’s FY 2024-25 Adopted Budget. Future costs of $64,508 will be covered by proportional contributions from all County Health divisions, which may include some Net County Cost, depending on the composition of each division’s contribution.
