BACKGROUND:

Paratransit services are necessary for County Health clients to travel to court hearings, care facilities, medical appointments, and other necessary appointments. These services are provided to clients of Aging and Disability Services, Behavioral Health and Recovery Services, Family Health Services, Correctional Health Services, and San Mateo Medical Center who do not have their own transportation. Clients using the paratransit services include older adults, adults with disabilities, families with young children, clients with bariatric conditions, clients with substance abuse issues, and those who need wheelchairs, gurneys, or medical equipment.

 

County Health selected Allied Transportation Services LLC and Serra Medical Transportation Inc. to provide paratransit services after a Request for Proposals (RFP) was released in October 2025, consistent with County of San Mateo Administrative Memorandum B-1.

 

The previous providers for paratransit services have reached the five-year term with their contracts but Procurement approved a three-month extension for contracts to be negotiated with new contractors. The terms of the new contracts approved here are expected to begin in March 2026.

 

DISCUSSION:

County Health received eight responses to its RFP for paratransit services and the evaluation committee recommended that contracts be awarded to Allied Transportation Services LLC and Serra Medical Transportation Inc. The proposals from both companies are “responsive” and “responsible,” as defined in Administrative Memorandum B-1.

 

These agreements will allow County Health clients to continue receiving essential transportation services. Clients travel primarily to court and medical appointments from board and care facilities, private residences, acute care psychiatric facilities, hospitals, and skilled nursing facilities. County Health staff are responsible for coordinating ride requests and monitoring service delivery to ensure that client transportation goals are consistently met.

 

Each agreement is for an amount not to exceed $390,000 for the term of March 15, 2026, through March 14, 2029. The prevailing rates are consistent with the rates in County Health’s current paratransit contract.

 

The resolution contains the County’s standard provisions allowing amendment of the County’s fiscal obligations by a maximum of $25,000 (in aggregate).

 

The agreement and resolution have been reviewed and approved by the County Attorney as to form.

 

It is anticipated that the two paratransit service providers will be available to provide transportation services for court hearings, care facilities, medical appointments, and other necessary appointments 100% of the time when services are requested.

COMMUNITY IMPACT:

The paratransit service contracts with Allied Transportation Services LLC and Serra Medical Transportation Inc. will positively impact County Health clients, including older adults, adults with disabilities, families with young children, clients with bariatric conditions, substance abuse issues, and other needs, served by several County Health programs. These clients do not have the ability to use public transportation due to physical, cognitive, or mental health challenges and do not have private transit to their destinations. These contracts will continue to ensure equitable access to paratransit services to clients of County Health.

 

FISCAL IMPACT:

The term of the agreements is March 15, 2026, through March 14, 2029. The aggregate amount of the agreement is not to exceed $780,000 for the three-year term.  Funds are included in the County Health FY 2025-26 Adopted Budget, and similar arrangements will be made for future budget years.
