BACKGROUND:
Measure K is the half-cent general sales tax initially approved by San Mateo County voters in November 2012 and extended in November 2016 for a total of thirty years.
The Board of Supervisors (Board) held study sessions on Measure K expenditures and approved funds for the fiscal year (FY) 2025-26 budget cycle, divided equally among the five supervisorial districts, for one-time district-discretionary needs and projects. District 4 has submitted a request to use their district-discretionary Measure K funds as shown below and described in the Project Summary section of this memorandum:

This item is consistent with the criteria for district-discretionary Measure K funds approved by the Board in December 2018. District 4 indicates that this organization is either a public agency operating within the County or a nonprofit organization registered and in good standing with the California Attorney General. District 4 indicates that the sponsoring Board member, member’s family, and member’s supervisorial staff do not serve as an officer, director, or play a policy role for the grantee organization.
DISCUSSION:
This is a request to authorize a grant to Ravenswood Family Health Network and resulting grant agreement, in an amount not to exceed $10,000, to purchase a fully automated advanced lensometer for the Optometry Clinic at the Ravenswood Family Health Center in East Palo Alto. The County Executive’s Office will administer and manage the proposed agreement.
District-discretionary Measure K funding for this organization has previously been provided as listed below:
• On February 13, 2018, the Board of Supervisors approved district-discretionary Measure K funding, sponsored by District 4, not to exceed $69,000, to Ravenswood Family Health Center, for the Dentistry Renovation Expansion Project.
Ravenswood Family Health Network is a nonprofit organization with a federally qualified health center headquartered in East Palo Alto that also operates multiple clinics in San Mateo and Santra Clara counties, serving primarily low-income patients. The organization provides comprehensive care, including primary care, women’s health, behavioral health, dental, and optometry services.
This district-discretionary grant will be used to fund the following project or services:
The Ravenswood Family Health Center’s existing lensometer has limited functionality and requires manual operation, which increases the risk of human error and limits accuracy, particularly for patients with complex prescriptions. Inaccurate prescriptions can lead to ongoing discomfort, reduced vision quality, and the need for repeat visits. This grant would be used to purchase a fully automated advanced lensometer.
This district-discretionary grant will enable the organization to meet the following currently unmet need or service gap within the County: Purchasing a fully automated advanced lensometer will improve the accuracy and efficiency of eyeglass prescription verification at the Ravenswood Family Health Network Center’s Optometry Clinic in East Palo Alto.
District 4 has informed the grantee organization that this grant is one-time only, and that the grantee organization will need to find other sources of funding going forward.
The grantee organization will use the Measure K grant as follows:
• The purchase of a fully automated advanced lensometer for the Optometry Clinic at the Ravenswood Family Health Center in East Palo Alto.
The project or services funded by this district-discretionary grant will be implemented or provided from April 21, 2026 to October 31, 2026.
Total Measure K Request: Not to Exceed $10,000
The release of funds will be contingent on the execution of an agreement providing for the County’s confirmation of the expenditure of funds for the purposes stated herein. The County will disburse the funds to the following organization for the purposes described above:

COMMUNITY IMPACT:
This investment supports equitable access to high-quality vision care for low-income residents in East Palo Alto and surrounding communities. By improving diagnostic accuracy, the new lensometer will help ensure patients receive correct prescriptions the first time, reducing unnecessary follow-up visits and improving overall health outcomes.
Vision care is a critical but often overlooked component of preventive health. This funding helps strengthen a trusted community provider and improves care for thousands of residents who rely on Ravenswood Family Health Network as their primary source of care.
FISCAL IMPACT:
There are sufficient Measure K funds for this specific FY 2025-26 Measure K request. These funds are budgeted in the Non-Departmental Services FY 2025-26 Adopted Budget.
