BACKGROUND:
San Mateo County Health has been awarded the Health Care for the Homeless/Farmworker Health (HCH/FH) grant from the Bureau of Primary Health Care, Health Services and Resources Administration of the US Department of Health and Human Services. The grant funds San Mateo Medical Center’s (SMMC) ongoing HCH/FH program, which is designed to assist homeless and farmworker adults, families, and youth with access to and utilization of SMMC’s continuum of care and other medical, health, and support services.
In January 2021, the HCH/FH program completed a Request for Proposals (RFP) for enabling services, which are non-clinical services that enable individuals to access health care and improve health outcomes. Enabling services include case management, referrals, translation/interpretation, transportation, eligibility assistance, health education, environmental health risk reduction, health literacy, and outreach. Abode Services was chosen to provide these services as a new effort for the HCH/FH Program, based on their efficiency in delivering enabling services to the population they serve.
In September 2021, the County and Adobe Services entered into an agreement for enabling services for the term of September 15, 2021, through December 31, 2023, in an amount not to exceed $343,748.
DISCUSSION:
HCH/FH program has determined the viability of these new services and is requesting to extend services by one year to allow the program to bring services into alignment with HCH/FH’s other contracted services. This amendment is coming to this Board late due to prolonged negotiations with the vendor.
SMMC plans to issue an RFP in 2024 to award contracts for services for the 2025-2027 period as directed by the HCH/FH Co-Applicant Board, in coordination with the 2024-2027 HCH/FH Strategic Plan.
County Attorney has reviewed and approved the resolution and amendment as to form.
The resolution contains the County’s standard provisions allowing amendment of the County fiscal obligations by a maximum of $25,000 (in aggregate).
The County’s Contract Compliance Committee has approved a waiver request for a 39-month term for this agreement.
It is anticipated that 100 unduplicated individuals experiencing homelessness will receive care coordination or assistance annually.

FISCAL IMPACT:
The term of the amended agreement is September 15, 2021, through December 31, 2024. The amendment increases the amount payable under the agreement by $150,000 to and amount not to exceed $493,748 for the 39-month term. Funds in the amount of $37,500 are included in the SMMC FY 2023-24 Adopted Budget. Funds in the amount of $75,000 are included in the SMMC FY 2024-25 Recommended Budget. Similar arrangements will be made for future years.
Expenses at SMMC are covered by fees for services or third-party payors whenever possible. The portion of expenses for services provided to the medically indigent or to those covered by programs that do not meet the full costs of care is covered by the County’s General Fund contribution to SMMC and is within the existing annual appropriation.
