BACKGROUND:
San Mateo County Health has been awarded the Health Care for the Homeless/Farmworker Health (HCH/FH) grant from the Bureau of Primary Health Care, Health Services and Resources Administration of the US Department of Health and Human Services. The grant funds San Mateo Medical Center's (SMMC) ongoing HCH/FH program, which is designed to assist homeless and farmworker adults, families, and youth with access to and utilization of SMMC's continuum of care and other medical, health, and support services.

In January 2021, the HCH/FH program completed a Request for Proposals (RFP) for enabling services, which are non-clinical services that enable individuals to access health care and improve health outcomes. Enabling services include case management, referrals, translation/interpretation, transportation, eligibility assistance, health education, environmental health risk reduction, health literacy, and outreach. Abode Services was chosen to provide these services as a new effort for the HCH/FH Program, based on their efficiency in delivering enabling services to the population they serve.

In September 2021, the County and Adobe Services entered into an agreement for enabling services for the term of September 15, 2021, through December 31, 2023, in an amount not to exceed $343,748.
